Put your feet up on a squatty potty while sitting on the toilet.
If you don't have a squatty potty then put your feet up on something that is 8-10 inches off the ground and it stretches you so that the rectal cramp subsides before you have a full blown episode.
It has worked for me everytime for 4 years now after 20 years of almost passing out, cold sweats, and incredible pain.
