Mysterious burning in penis

Hi to anyone reading this, I have joined patient info for help as I am desperately needing to get rid of this burning feeling.

A little about me, I am 32 Years old, and feel like my health is a slippery slope slowly sliding downwards for about 2/3 years, and I'm slightly overweight (not obese). I eat mostly healthy home made food with veggies very regularly. I drink squash religiously and coffee (tried decaff for 2 months with no difference)

So firstly lets start right off the bat I have diagnosed Peyronie's Disease as of October 2019 (around a year so far) I believe I know the ins and outs of Peyronie's, I do suffer with some discomfort but manageable, I have also been using a vacuum device to reduce the bending (only once after the cystoscopy)

About 6 months ago started a burning sensation along with smellier than usual urine, increase need to urinate, and increased urgency, I thought this was a UTI and so did my GP so I was given antibiotics for a UTI by my GP, within the first day of taking the antibiotics the burning was gone so I was a bit confused as I thought it takes days before it kicks in, I took the full course anyway but after a week the sensation returned along with all the other symptoms.

I went back to my GP and has a urine test which showed up negative for a UTI, so requested a prostate examination as my urine flow was feeling way less almost like I have no ability to urinate with force like I previously could, and I was worried it couldn't be something sinister like prostate cancer, the examination was clear, I also send in a urine test, a blood test and a STD test which all came back clear. so I was referred to a specialist (the same Dr who diagnosed my peyrionies).

I failed a urine flow test with the reason that my bladder is not clearing properly.

I have seen the specialist and had the horrible experience of a cystoscopy which was painful to say the least, and I feel a lot has not been the same since the examination.

The cystoscopy came back with nothing too worrying, and the specialist said I have in his words "a short prostate with a high bladder neck, which is likely the cause of the urine flow symptoms", I have been prescribed with Tamsulosin to help, but is not helping at all.

I also don't understand this conclusion / diagnosis, if I have a high bladder neck and a short prostate, surely this didn't get like short term along with my symptoms so I'm going to list all my symptoms in detail below in hopes someone has something different in mind.

  • I have burning sensation
  • the burning is almost always non-existent in the morning
  • the burning gradually gets worse throughout the day and can be pretty intense by the evening.
  • it does not hurt nor burn any more than usual to pee
  • after the cystoscopy I have a slight discomfort from what feels like my prostate when urinating and ejaculation.
  • ejaculation frequently intensifies the burning sensation
  • my urine does smell really very strong of ammonia especially in the morning.
  • I'm not 100% sure on this but sometimes I feel like I can urinate more easily after defecation.
  • I have way less control over my urine flow, in fact I feel like can just let is come out how it comes out but i cannot force the flow faster or slower any more.

All of the symptoms above have come and gone 3 /4 times each time lasting at least 3 weeks. and going away for maybe a week at most.

please someone help me with this horrible sensation, its affecting my work, my well being, my self confidence and my general health.

    In what country do you live Louis?

    I would say that if you want to bring about change you need to d something radically different.

    Trying something for 2 months and then stopping is not part of this. You need to change a number of things, such as you diet, completely, and change it for 6 to 8 months before you can really tell whether you are going to get long-term benefit from it.

    So you say " I drink squash religiously and coffee " - would you be prepared to stop these for 6 to 8 months - I would say that when you are dealing with UTI type symptoms that have been ongoing for some time that the first kinds of things to avoid are coffee and squash. You need to drink plenty of water everyday, the suggested amount is 2 Litres, but part of this can be from fresh fruit juices as long as they are not made from concentrate.

    As part of my liquid intake I often have coconut or almond milk with my cereal for breakfast.

    You may need to seek the help of a dietician - but there are loads of free sources of health diet advice online and in health magazines. You have had this issue for too long, unfortunately often you need to put in some concerted effort to get the outcome that you want - this often means radical change to what is happening in your life - not just seeking medical profession answers from people like GP's and the other specialists you have seen.

    You do not mention whether you are an active person, or perhaps were prior to the COVID situation?

    And just to clarify, have you actually seen a Urologist, or perhaps more than 1 Urologist?

    Despite what you have going on, such as the peyrionies and all the other symptoms and test results, you still seem to be sexually active on a regular basis. Is this a fair assumption to make? If you are, would you describe the sex you have as satisfactory or are there complications of the symptoms you have involved in this too?

    I see one of the symptoms you get is after you ejaculate this often intensifies the burning sensation - having this occur is quite normal anyway, but if you have a dietary imbalance you may be getting this far more severely that you otherwise would.

    One last question: from all the medical professionals you have seen is there one of these you feel you could create a working professional relationship with to find a solution to what you have going on? This may be a GP, or some other specialist .....

      So firstly I live in UK.

      I have been sexually active and completely faithful with my wife but this is of course drastically reduced since my symptoms, maybe once or twice, also after this activity its like a huge amplifier for the burning so its not really desirable until I get some resolution, the curve I have developed is less severe, having used soma correct I feel it made a very large difference.

      Apologies for my badly worded post on my drinking habits, I religiously drink squash (water with a splash of squash for flavour) and I drink easily around 2L of this per day, I also have coffee a couple of times a day ( with oat milk ) I have tried going without and I have tried going on decaff both of these.

      I am also completely for making huge dietary changes, I have been suspicious of our water from the tap in our house we live in for many years so i have even resorted to buying water and completely relying on botted water to see if this made a difference ( hydration wise i feel it has, symptoms wise it has not)

      I strongly believe my diet is not an issue, I joined my wife on keto several times and have already made drastic changes, I have also taken to mass reducing my meat intake which we have maintained, we eat very healthily, we make home cooked food fresh almost every day, I also have done keto during the course of this issue without any change either.

      I have now gone back to my GP and they have given me antibiotics for prostatitis, currently i'm 5 days into the course with absolutely no effect on the burning or urine retention which seems to have kicked up a gear on severity this week.

      So my activity since covid is dreadful, especially as I work in IT, this is mainly because I fear I have this horrible problem the last thing I need is to catch this dreadful virus so I have not been going to the gym, I previously was going to the gym 3 times a week and I do an hour session of lane swimming (and I don't mean just swimming around, I make myself very hot and out of breath)

      So I have been to see the urologist who gave me the cystoscopy, but honestly I don't really agree with his findings, and unfortunately my main GP has just retired and they are "in the process of assigning me a new GP" not that I ever managed to get an appointment with my assigned GP anyway, I cam terribly dissatisfied with Wokingham medical centre to say the least.

      whilst I am grateful for your advice, I feel If I try to make a change to figure out this problem for 8 months, It would take far too long to find the root of the problem, I have had this burning for 3 months I could not possibly take another 3 let alone another 8 trying to figure out this solution, I need results quicker, this problem came almost overnight, surely there is a way to resolve it in a shorter time frame.

      I am hoping to create hopefully that relationship here, to get to the bottom of this, as i cannot really rely on my medical centre as they make me speak to someone different every single appointment,

      The best hope you have I would suggest is to form a working relationship with a Urologist - there are very few GP's that would have the specialist knowledge to support you in a meaningful way to overcome what you have going on.

      A Urology department at one of the Largest teaching hospitals is perhaps the best place to look - you could initially visit a GUM Clinic at the same hospital.

      You would need to explain your situation and state (politely of course) that you would like to resolve your issues as soon as it is possible to do so and ask if it would be possible to get to see one of the Senior Urologists based at the hospital.

      Sorry, but I would suggest there are no 'quick fixes' for what you have going on - this is why it is important to build an ongoing working relationship with a medical professional who is prepared to see this through with you until all the issues are resolved in the best way possible.
