Nausea since start of 2022, losing weight, GI specialist puzzled - I just want to feel well again

Hi - I'm female, 58, vegan for the last 5 years, and since the beginning of 2022 I have been suffering with nausea, occasional vomiting, bad heartburn, and difficulty sleeping. I've lost 25lb this year because I can't eat much at all, even with making an effort to eat more calorie-dense foods.
I've had heartburn on and off for the last 30 years or so (since being pregnant) and was on omeprazole 20mg/day until the beginning of March, when it was increased to 40mg/day which didn't help (I am a rapid metabolizer of omeprazole anyway) and then at the end of March I was switched to famotidine 40mg/day which helped a lot with the heartburn - I'm now on 20mg/day - but the nausea remained.
The nausea is worst at night; I used to sleep on my left side but that makes me feel worse so I sleep on my back now, with an extra pillow. I get awful lurching sensations in my stomach as I am dropping off to sleep, which have the effect of waking me up completely and sending me into the bathroom as I feel I am about to vomit. Occasionally I do; more often I just stand in the bathroom miserably for 20 mins or so and then return to bed and sit up and read for an hour or two until my stomach has calmed down.
I've had various tests so far, including blood tests, stool test for H Pylori, gastroscopy - all normal. I had a CT scan which appeared to show an inflammatory ileal stricture, which was worrying, but a follow-up MRI scan showed no stricture but many adhesions in my abdomen (a result of 2x c-sections with infections after both, and a hysterectomy).
I'm currently taking domperidone 10mg 2x a day and cyclizine for the nausea; I am eating my evening meal earlier and earlier but still have 3 or 4 bad nights a week. I physically can't eat very much food - am maybe eating half what I used to, on a good day, and on a bad day, even less.
