Need help had an anal fissure for 2 months and my country has no Nitroglycerine ointment

Hi there I am a active 33 year old male who recently lost a lot of weight and am down to 215lb now about 2 months ago I took a very hard s**t I was constipated and I forced it dunno why I did that.

I am only getting GTN in the form of tablets or a SPRAY. Can I use the spray if I can't get the cream?

Anyways when I had this constipation it caused a bad tear and have had fissures in the past that healed quickly but not this one, this is my 4th one in 2 years. This one doesn't seem to want to go away, after I take a s**t there is this insane lancing spasm that has brought me to near suicide on 2 occasions this week. Luckily I took an 800mb ibuprofen and this worked like MAGIC. I only need to take 1 in the morning it seems and I am fine for the entire day.

The only success I have had was eating just 1 cup of blended oats and chia seed with soy milk and straw berry in the morning and eating nothing for the rest of the day, then I applied Zinc Oxide 40% inside the anus and this made me feel perfect for 4 days.

Then I thought I was cured so I ate a big slice of watermelon Sunday, this was a huge mistake. The added amount of fiber made the s**t massive that tore my asshole apart. Something to note by doing this I only get a bowel movement once every 2 days due to how little amount of food this is. This seems to be the key to healing my fissure. I think I will get coconut oil and put it in the fridge to solidify and insert the solids and see if it will also help. One thing I notice it itches like crazy with the Zinc Oxide 40% this is a sign that it is healing because healing wounds always itch like mad.

I no longer getting any blood so maybe this is some progress? but the throbbing pain after the bowel movement is insane. Especially as I ride a bicycle to work 6 miles. I nearly killed myself yesterday by drinking poison due to how bad the pain was then my driver whom I work with gave me a 800mg ibuprofen and this stopped the insane pain within half an hour.
