Nerve Pain after varicocele surgery

My son (40 years old) had a varicocelectomy eight months ago . He has been in terrible pain since. (nerve pain around the incision areas both sides of the groin at times going down to his testicles). He has seen a pain management doctor and was prescribed Duloxetine and was given nerve block shots. Nothing helped. Has anyone been in a similar situation? and what has helped you? He is desperate for any relief.
