Never been able to masturbate or ejaculate

I am 28 years old, exercise regularly, eat extremely healthy, do not do drugs or smoke, drink occasionally, normal blood pressure, no diabetes, testosterone level is 937, all blood work has returned normal. I have never been able to masturbate or ejaculate in my entire life due to almost never being able to get an erection. The few times I am able to get an erection, it only lasts a minute or two, and it is never rock hard. The erections are always extremely painful. My doctor has me on Viagra and Terazosin but it has not done anything. I have tried fleshlights and penis pumps, they both cause incredible pain instantly when trying to use them. I am at a loss as to what to do. The doctors have no answers. What should I do?

    Hey buddy I feel so sorry for you.  How long have you been this way.  Why did you want so long.  You have a blockage some were that the  blood is not going into the penis  Terazosin does cause pain full erection.  I think you need to see another Urologist.  You need one that specialize in fertility.  The blood is not going to the penis.  He would be able to check.  Do not want to long.  You need this fix not.  I am going to send a e-mail to my doctor to see if he would know what is going on and to make sure I am telling you the right thing  Ken

      It's been this way my whole life. I've had a rough life, haven't really had access to doctors and specialists until this year. And I haven't had a girlfriend until now so it was not a priority on the rare occasions I was able to go to a doctor. There were always more pressing issues.

      Good morning buddy.  Sorry for what you are going through...I have not heard from my doctor yet but what I told you before is see a Urologist that specialize that deal with fertility.  They know more about stuff like that.  Glad you have a girlfriend.  You need to try to relax.  Stress is not good for you and That may be one of your problems.  Now this problem should go to the head of the class.  Good luck  Ken

        That is great.  I do believe you have some type of problem and that Urologist will get get to the problem.  But you need to relax and have some fun and not think about it  Stress will kill a lot of stuff also get some sleep so you body can heal itself.  We all the guys on here we are with you and we will try to help.  Take it easy buddy and don't force the issue.  Ken

    As orgasm and ejaculation do not require full erection, you should be able to produce semen. Has this ever happened?

    The pain on erection is not at all normal, so surely more medical investigation should be possible. It would be worth pushing for scans to check the physiology of your penile blood vessels. Maybe there's some major block in the arterial system of your penis. If the blood vessels are OK, maybe there's a nerve problem. Have you had any spinal injury?

    If you hope to have children, there would be ways of collecting your semen medically using one of 2 methods.

    • Penile vibratory stimulation (PVS). With this method, a vibrator is placed at the tip of the penis, triggering a reflex ejaculation.

     Rectal probe electro-ejaculation (RPE). RPE is an option for men who do can’t ejaculate through penile vibratory stimulation. In this procedure, a doctor inserts an electrical stimulation probe through the man’s rectum. In a controlled manner, the doctor uses this probe to stimulate ejaculatory nerves. Usually, the probe is in the rectum for about ten minutes. Some men need anaesthesia for this procedure.

    I hope you find some answers as your situation is very detrimental to any man's quality of life.

      No it has never happened. I have not had a spinal injury. I asked my urologist to do an ultrasound or any other test for that matter but he did not want to.

      Do you have any other circulation problems eg cold toes and fingers? I guess it’s not nerve damage, as you say you haven’t been injured. 

      It doesn’t seem acceptable for your urologist to say he doesn’t know what to do. He should get advice or refer you to another specialist. Which country / health system are you in?

      I do have blood circulation problems. Oddly it does not affect my blood pressure but it does cause my legs to ache when not enough blood gets to them. It might be nerve damage, as I have fibromyalgia, so I know my nerves are not normal. I am in the U.S.

      Sad to say, I think your circulation and general health issues may explain your sexual function problem. It still should be possible to be able to have a family, but I wouldn’ t hold out too much hope for having a fully normal sexual experience. Best of luck to you whatever happens.

      Plysi.   Did a little looking on the internet.  Fibromyalgia can cause sexual problem.  How is you libido.  What type of medication are you on for it.  That medication could be your problem.  But please see the doctor that I told you to see.  I think they can help you.  You need this taking can of you have a long life to live.  Ken

      My libido is extremely high. I am on Lyrica. I will try to do what you said and see a urologist that specializes in fertility. However I have Kaiser as my insurance so everything has to be in house and they are extremely uncooperative in everything I do with them. I will try my best though.

    By you saying "the doctors have no answers" I wonder if you have ever been referred to a Urologist?

    If you mean your regular GP / Family Doctor / Regular Practitioner I would simply assert that these people do not have the expertise to deal with what it is that is causing this issue for you.

    Depending on which country you are in will determine what level of Urology expertise you have potential access to.  If you are in the UK most large cities will have sufficiently large (an therefore the expertise) Urology departments based at one of the main hospitals.

    If you are dealing with a GP ask for a referral to one of the main Urology departments in your area.  Also, many sizable cities in the UK (and elsewhere in the world) will have specialist Sexual Health Centers where you simply find out where they are, give then a ring and say you would like to see one of their duty Urologists and find out the sessions and days when you can access one.  (Practicalities / Logistics of this may mean you take a day's holiday from work, or whatever.) You must assert that you want this sorting once and for all and that if you need to see one of the top UK Urologists at a UK Urology Centre of Excellence then this is what you would like them to arrange.  Ask where the Centre is based as you may have to arrange a referral via your GP - PLEASE be aware ALL NHS patients have the right to access a Centre of Excellence, no matter where it is in the UK.

    There are other physical approaches you can take - I certainly would have tried a penis developer and fleshlight as you have done - BUT, only a Urologist can tell you whether you are at great risk of damaging whatever it is that has caused this problem for you.  Often, many medical professionals would be loathe to suggest you try different things yourself in case the worst possible reaction does occur ...

    So, try getting to see the top Urologist that you can possibly access - whether in your area, or a one you need to travel to.  Often, asking a Urologist something like this: if this was a close family member of yours what advice would you give them as to who to see and the steps to take to have the problem sorted!

      I have gone to a urologist, he is the one who gave me Viagra, Terazosin, the pump, etc. But he said he doesn't know what to do about the pain besides taking Lyrica and Motrin, which has not worked at all.

      You need to see a Urologist who knows what they are talking about, rather than give you lame-excuses!  Any specialist who is worth their salt would not simply say 'they would not know what to do' - any professional with any credibility would be sufficiently up-front to state they do not know themselves, but someone else they can name from their professional Urological organisation would know how to sort your problem, and, give you that Specialist's contact details.  Very poor!!!

      Can you get to see a Senior Specialist Urologist in your local area / City? - You can usually find one that lists what specialisms they deal with  - your may be described as Impotency issues  ......

      I have to see providers that are covered by my insurance, and that is a very limited list. I will try and find a good urologist.
