Night Pain

hi everybody I started taking my pred at night as I wasn't sleeping due to pain I was currently on 6mg at this point, anyway this seemed to help for a week or two , I'm currently using the slow method and I'm on 3 days 5 1/2 and 4 at 6mg this week but I'm waking up in the early hours aching, this morning it was all down both sides of my back and across my shoulder blades, the thing is once I'm up and about this pain goes so I don't think it could be the PMR especially as I say I now take my pred late evening

hope someone can help me with this .

Best wishes Molly

I've been splitting my dosage ever since I started (2 years ago) at 16 mg Medrol/day; 8 mg in the a.m. and 8 mg. in the pm.  I am currently at 2 mg/day; 1 mg in the a.m., 1 mg in the pm.  However,  I am now stuck at 2 mg/day for over 2 months and can't seem to be able to reduce even by 1/2 mg.  As soon as I try, pain returns.  Now this morning, my right ankle is hurting (even on the 2 mg dose).  It could be the high humidity, but I'll see if my ankle improves over the course of the day.  So frustrating because I've been trying to walk for 20 minutes/day and I can't now with a painful ankle.

I was a bit worried about splitting mine as I take the full dose between 10 & 11pm but I'm in pain a few hours later but fine through the day .

You have to find the right method that works for you.  Splitting the dosage am/pm has worked for me thus far.

Molly - 'We are all different' and you will have to find the best method that 'works' for you.   It also depends on the type of tab you are taking - the white - non-coated ones work a lot quicker but can cause stomach problems.  The red or colored ones are enteric and don't dissolve in the stomach and take a bit longer to get into your system.  An example  would be the white ones early with breakfast (or split with Supper) the enteric ones late in the evening so they 'work' by early morning.  On no account ever cut the enteric version!  There is a member on the forum who purchases gelatine capsules  and put the white crushed ones into them.

I sometimes get minor aches during a reduction which usually goes after a few days but if it persist stop reducing and/or go back to your last dosage that 'contained' the symptoms.  Moving will help and gentle exercise

I'm in the uk and don't think they do the red ones now mine are the white ones , I'm just worried that the dose I'm on now isn't lasting through the night so if I take less in the evening it might be worse .

It could be steroid withdrawal symptoms, which I've found become more troublesome and harder to sort out from true PMR symptoms the lower the dosage.  I would suggest you not reduce further for a few days, and see if there is any improvement.  Also, try to get a bit more rest.  You're at a dose where the adrenals really have to kick in and they can be, as I've learned for myself, painfully slow to get up to speed.  You still have a serious disease, so be extra kind to yourself.  As EileenH says, it's not a relentless progression to zero.  Slowing down right now could mean greater success, and a lower lifetime total pred dose, in the future.

Molly this is very good advice from Anhaga.  I and many others have found it very difficult to judge dosage during reductions hence the reason of doing the DSNS method.smile
