no quad control following arthroscopy

hi there

had a routine right knee arthroscopy under gen anaesthetic in nov 2012. 2 weeks later on removing dressing and stiches, physio noted i was unable to do a straight leg raise. was sent to physio, had lots of tests and 2 years later am still unable to do a striaght leg raise, use stairs properly etc.my knee gives way constantly and as a result i ended up with a stellate patellar fracture and patella baja., after a bad fall 12 months ago..i have been in a leg brace for 6 months and see a physio but there has been no change in my quad. my quad has atrophied and i have no motor power. there is no sensory loss however.

any advice /?/ anything at all???

    The theory that made sense to me was that the brain shuts off receptors for the area because of the surgery trauma and pain. I heard that it must be dealt with quickly within a few months or it becomes permanent. We were able to resolve this issue within the 3 month timeframe for our daughter who had youth on her side by doing clam exercises, sitting with back against wall and both legs straight and firing both quads at same time (it helps to work both sides together), massage, and a change of scenery (we went out of town to visit family and it was amazing how the change of environment stimulated her brain).  So sorry for your challenges.       Oh so your daughter recovered from this. Great for her.  I misread.   I do clams with black rubber around quads and many other stretches. I try go out more don't want to stay home with cabin fever

      yeah...mine wasnt dealt with quickly enough i think...my quad has completely atrophied and wont even contract with an electrical machine

    Hi,  I am experiencing exactly the same thing and surgeons are baffled with me too.  I was extremely fit,  no problems with muscles, tendons, ligaments.  Had simple right knee arthroscopy to clean my knee joint out, scraping out inflamed plica to be exact in September 2015, after initially hurting it back in January 2013.  I was unable to squat or fully bend my knee, and had excrutiating pain at night time while sleeping.  After the arthroscopy, I could not walk on right leg after this, struggled for months and months to work out a way to walk without my knee buckling on me.  February 2016 surgeon sent me for an mri as my knee had an indent evident and he felt I had a torn patella tendon which was why I could not walk properly and my knee was buckling if my knee had weight on it and bent.  I could not do a straight leg lift, walk up and down strairs, could not flex my thigh muscle at all.  MRI showed all okay.  Quads kept wasting away, all my research showed tendon issues. August 2016, I requested from my GP to send me for a ultrasound on my knee tendons as it was nearly a year now, with severe quad wasting and my surgeon was doing no follow up work to try and work out what was going on.  Ultrasound showed patella tendon laxity. Surgeon had me in for surgery November 2016 and did a Tibial Tubercle Transfer to tighten the tendon. Was in a brace for 6 weeks.  Started Physio end of January 2017 and was excited I could flex my muscle after a couple of weeks.  Could do little amounts of leg lifts a month later, then 3 months later physio exercises started becoming very painful.  Physio sent me back to the surgeon, xray was done which showed my patella had dropped further and now had patella baja.  Surgeon felt there was nothing else he could do, said to handle the pain as much as I could, stop physio exercises cause the bone was rubbing on bone, and even though I had nothing wrong with cartilage, eventually I would have to go back to him as the cartilage would be diseased and he would then deal with it.  I went for 2 other opinions.  My new knee surgeon wants to move my patella back up 2.2cm and place another 4 screws in.  I asked him wouldn't it then make my patella floppy again by moving it up, he said we will deal with it when the time comes.  I wanted my quad muscle to be scanned to see if there is an underlying issue there seeing I've gone backward from the TTT surgery. He doesn't seem to think that would be the issue.  I was concerned that after the original simple knee arthroscopy, my knee may have been bent fully while I was still on surgery table to make sure my knee was working properly, and seeing I hadn't been able to bend it for 2 years prior to the surgery, did something happen that snapped something in my thigh to cause constant ongoing problems.

      I am now 2 years after my surgery. Few months ago I was able to finally do a leg raise. 3 weeks ago I'm able to do quad raise under a roll however still have pain under my knee cap. I'm taking duloxetine and that masks my arthritis pain.  I still have stiffness in the knee. My leg is still weak.  I'm going to surgeon today too see about the patella pain.  Patience and time I'm not sure how long a weak leg takes to recover.  It's a long recovery process

      i too have patella baja....and still cant lift my leg at all. not even in the slightest!!1

      in a KAFO leg orthotic

      all specialists are baffled!!!!

    I am 4 week’s post op now of a meniscus repair.  I zero control of my quad. I can’t do a leg lift to save my life. If I was my knee buckles.  Seeing that I am not alone brings me a little piece of mind but also worries me that this could be a much longer process.

      are you able to lift your leg in the slightest?? i know i cant...so i cant even do the recommended exercises....and hey.....like your user name....boeing 74...you in the aiRlines???

      I can’t lift it even the slightest. To get in and out of bed or even out of the car I pull it with my left leg.  I am in the aviation industry!!

      i too cant lift mine at all....so cant even do quad exercises as it just will not work!! its so frustrating isnt it?? what have your doctors said?

      i was working for an airline for 20 years before this op!!!

      my knee gives way all the time and im in a leg brace!!

      Oh my goodness I’m so sorry your career had to end. I’d be lost to be honest with you.   If this seems to be an ongoing issue for me I’ll get a wheelchair and get back to work. I love being around airplanes too much.  

      Your leg brace, does that allow you to walk without assistance? 

      The physical therapists are stumped they don’t understand what is happening. They use the electric stimulation (russian) and nothing happens. Not even a small little twitch.  I can’t do any type of quad exercise I try and nothing happens, I can’t even lift my leg high enough to get in a car. So far that’s the only thing they have really tried.  

      I have a follow up with my surgeron on the 28th and I’m sure we will discuss this then.

      wow thats so like me....they all keep saying they have never seen it before....and same here with the elect stimulation....no muscle twitch at all in right leg...and no knee relex

      i have crutches but only use one

      i had a really bad fall once though as the knee keeps giving way....and broke my kneecap...on top of all this!!!

      are you in uk?

      i would be so interested to hear what happens on 28th and what your surgeon says

      I am in the United States. It’s crazy to see someone else with this same issue. I honestly thought I was the only one.

      Thankfully my knee can bend and straighten but in the mornings is very painful and stiff. So at least my hamstring is engaging but there is nothing in the quad.  

      When did they start to worry or question this? I am one month post op today.

      Good Luck it's a long road to recovery. I can do leg raises now. Go on disability watch your mental illness you will isolate yourself in time.  In time with hard work your leg raise will happen.  Forget Russian for now. Use for pain.  Lots of heat before exercises.  Good luck

      Thank you. This was an on the job injury that lead to this complication so at this point I have to do everything work Comp and the medical team say.   So far no one has been able to give me any type of idea what’s happening.  For now I just sit alone  (while my wife goes to work) trapped in my house till therapy where a stranger who speaks little English drives me to and from.  

      I’ve noticed my mental health is suffering for sure. I’m not argumentative and angry.  It’s hard for me to do anything and that is just taking away any good thinking.

      Is your entire leg weak yet from hip to knee. Also did you have a meniscus tear.  Russian tens waste of time. I have lots of experience with this as I have gone through it.  Are you limping badly yet.  What exactly did surgeon do to you.  I'll give you a few pointers.

      Medial, knee cap, lateral compartment and back of the knee where is your pain.

      Get voltaran use with ultrasound. You will probably fill with fluid known as per ansine bursitis.  You must work on the inflammation fluid that moves around different parts of knee  you should be able with light touch felell the sore spots. Believe me you don't need to apply lots of touch bursitis is very painful at worst

      I had medial and lateral meniscus repair. With a harvest of cartilage. My calf and hamstring are both getting stronger daily and firing normally.  I cannot walk without my knee buckleing.  I am still on crutches and cannot walk without it. 

      I have very little swelling now.

      Boeing74, Has anyone suggested doing a nerve conduction study to make sure your femoral nerve is OK?

      Next time you see your surgeon ask him if you have any OA in your knee. IF you do your  in for the long haul.  Lots of inflammation bursitis. If you have OA inside medial like me after meniscus repair you will limp badly. The fluid travels all around.  THere is lots to know.  You might have a bunch of knots . I'm no thanks sure but first step ask your doctor do you have any OA.  I'm sure you do. That's why you quads went weak.

      it really is crazy to see someone with the same issue....mine started when i had the stiches out and dressings off....10 days after op....they asked me to lift up my leg and i couldnt...but they said this was normal after an arthroscopy and to have 6 weeks of physio and it would start firing again!!! but it never has....and now everyone looks like a deer in headlights and they dont understand it...i have had many xrays, mris, nerve conduction studies......it seems the nerve is intact but not working!!!!

      am so fed up...and its so frustrating

      keep trying new things and nothing seems to work....and they have no idea why or how this happened!!! x

      Kittylamb, So the nerve conduction study you had was normal?  I'm not sure what you mean by intact but not working.  Is the nerve firing as it should?

      Hi. I'm really sorry you're going through this. Did you have a meniscus trim or an actual repair (where they go in and stitch the meniscus inside the knee)? If a repair, from what I understand it's supposed to take much longer to heal and being on crutches is normal for one month or even longer. Is a Harvest when they go in and plant foreign meniscus tissue into the knee? If so, It's possible the combination is just overwhelming your leg/knee. It could easily also be that your quadriceps muscle did shut down due to the surgery and may take a good amount of time to reactivate and get stronger. As you can see on this forum, everyone has their own unique set of issues from their surgery. It's been 2 years since my knee arthroscopy and I am doing better now, but I'm still not walking normal. I may have excessive scar tissue and or swelling, plus weakness in quad. My physiayrist said my Vastus Medialis (a part of the quad muscle) is considerably weaker and I've read that this surgery can cause weakness to that particular part of the quad. I would look up you tube videos on how to especially strengthen the Vastus Medialis muscle more, along with the whole quadricep, glutes, etc.

      Do you have a hard concrete feeling that you feel in your knee? That can be swelling inside the knee and is more common from knee replacements, but arthroscopic patients get it too. Studies say that even a small amount of fluid trapped in their can weaken the quads.

      Although, I haven't gotten mine yet, my surgeon told me to get an excerise bike to strengthen the quads.

      For your mental health, go easy on yourself... pamper yourself. This is the hardest thing ive gone through . To not be able to walk suddenly is a nightmare. My doctors just kept telling me to get physical therapy and it did help so felt, but I knew it wasnt that simple. I felt so alone because doctors weren't giving me concrete answers which made everything worse.

      This happens to men as much as women from this surgery. Two men at my church had considerable weakness from their surgery. One got better, but still struggles with some weakness 5 yrs later..the other had a lot of weakness for months but eventually climbed to the top of Mount Shasta! It depends on a lot of factors, so don't blame yourself.

      A friend of a friend has had to use a brace for a year, and a woman's daughter that I know only began walking well at the 2 years mark. For first year, I was dragging my leg behind me and literally forgot how to walk, but now I can bring leg forward and it's much better than before, but still somewhat weak. My energy is better now...first year and half was the worst ( year went by both slowly and quickly..).

      Really treat yourself... and allow yourself all of the sadness , bewilderment, and anger too. Its normal to feel all of that. Walking is a huge deal. Be well.

      this forum is great to see that there are similar cases...as every specialist i see say they have never seen it before!!!!

      im not sure either....its just what they keep telling me....they think that originally the nerve was compressed or severed but it has grown back but doesnt seem to be working...i.e not firing...

      the nerve is there and has regrown etc but not firing at all

      one consultant said it was like having the tv all plugged in but when you go to turn the on/off button on the tv set...nothing happens x

      My PT moved asked me to move my follow up with my sugeron.  I go on Tuesday now and depending on the visit I might be scheduled for a nerve test.   Still no type of improvement on my quad. 

      The good news is I’m finally off all pain pills except tylonal every now and then!

      im so gutted by all thats happened....was working as cabin crew for british aorways for 20 years, a job i adored...and as i say, lost my job due to all this

      Oh man speed bird?!? Wow I’m lucky to just know someone who’s worked for them.   I’m so sorry you are no longer able to fly. I know how much you miss it.  

      I’ll update on Tuesday after the appointment for sure.  I’m not confident they will understand or even know what to do next. The reviews of the surgeon are not the best.

      yeah....def give an update.....all info is good

      and on the aircraft note....my first flight was on a classic 747....and i was also licensed on 777 and 767.....oh and a dc10 when i started

      thought you would like that info being an aircraft buff like myself

      If your surgeon says you struggling and suggests cortisone shot tell him no thanks. Waste.  Just bad side effects. Tell him you'll follow up in 6/8 weeks.  Synvisc waste too. Time is your best remedy to healing

      Oh wow!! I love the 747 and the triple 7!!!  Gosh Boeing is such a great aircraft.  I’m sorry you’re unable to fly.

      I Have never had progress with cortozone shots. However with it being a work Comp injury i have to play hybrids their rules

      My surgeon has no idea what has caused this. He said the surgery did not have any complications.   He is going to order a muscle stimulation brace that is used with people who have Arthrogenic Muscle Inhibition.   So really I have no answers. He wants me to continue PT in hopes that my quad will wake up.

      i tried muscle stim but didnt work for me

      and me still seeing physio

      we have very similar cases which i know isnt good but at least i dont feel so alone...and unusual or unique as the consultants say!!!
