Non-specific dermatitis (lasted 9 months now)

So I have had constant burning, redness, itching, and general soreness/inflammation of my glans for a while now. All red and irritated under foreskin as well. This has been bothering me ever since i had chlamydia, but that's been cleared since February this year. Doc said probably balantitis, so i had oral antibiotics, oral fluconazole (antifungal), topical antifungal as well. No improvement from any of this. I use no soaps, chemicals, body wash, or other irritants that i know of. I use hypoallergenic detergent as well.

So recently had a biopsy, and it shows non-specific dermatitis. Which means pathology didn't find yeast or bacteria anywhere I suppose.. don't know if that rules it out. I have had bacterial and fungal treatments so I really don’t know what else would be triggering it though. Saw a dermatologist again a couple weeks ago, he says not to do circumcision yet and to keep hanging in there but it burns so intensely all day. very sore at this point too. i'm scared to try it anyway but I'm at my breaking point and have read in some cases that's all it takes for the irritation to leave. He said psoriasis or nothing typical jumped out at him after observation or seeing my pictures (way worse looking because erect), but that it could he a form of either contact dermatitis, allergens, or possibly a skin condition. Gave me Mometasone Furoate 0.1% (corticosteroid ointment) to apply for inflammation or in case its a skin condition but i have noticed no difference so far. Maybe even drier and more irritated than before. But also since this started it hasnt "flared up", its just been consistently the same every day no matter what lengths i go to to stay dry and free of any irritants. but surely the dermatologist would be able to identify lesions in inverse psoriasis, even if they aren’t typical plaques. No flaking or actually bleeding that i can tell.

So I guess I’m just curious if any of you deal with genital psoriasis, forms of dermatitis, or have had balantitis for a long period like this and had success in treating any of them Or if you can tell from my pictures and symptoms what you think it leans more towards?

thank you guys. i’ve been abstaining from anything sexual for months and have seen no improvement, it’s affecting me being able to work my regular hours and just my overall mental health at this point, because I've seen every doctor around here i can afford (KY), and i honestly dont know to try and moisturize it with aloe gel or a vanicream skin cream, or coconut oil. Or to leave it alone. My urine burns it pretty bad when it touches the glans, and urinating is uncomfortable in general sometimes. I dont know what to try for a barrier either.

    I think the best thing for you to do would be to find a Dermatologist that specialises in Genital Skin Disorders / Genital Skin Conditions.

    You can do internet searches for such a specialist in the area in which you live, the region you are or that you can easily travel to.

    Or, if it is difficult to find any you could check out web sites of professional bodies and associations that the majority of any practising Dermatologists are a part of such as American Academy of Dermatology

      A Google search doesn’t turn up any of those specialists in or near Kentucky, and I have asked other physicians (mainly dermatologists) about it for months and they act like it’s not a real thing. As far as the website, it doesnt really help me determine anything. if I could just figure out whether it’s a skin disorder or balantitis.. but nobody can tell me what they look like. was hoping maybe somebody on here had been diagnosed and successfully treated for one or the other and might be able to help me figure it out.

      Have you actually had more than one dermatologist do a visual on this?

      The other route you can go down is with a Urologist - if you can find a major practice near you that has other health specialists they may have a Genital Disorders Specialist based in the same facility and refer you immediately to a colleague.

      Having looked at your photos, and yes I see hundreds of photos every year, it does not show anything specific - with photo 3 showing the biggest potential for a concern being present.

      I would not just try any cream or oil, what you would most likely get is a negative reaction, and potentially a severe reaction. This is where the Genital Disorders specialist would advise you the best.

      When you say:

      ... I take it the detergent is for washing your clothes AND towels with? I would check, you seem to be minimising exposure to chemicals, but is there any contacting going on that you had not thought of?

      Have you seen more than one Dermatologist?

      Hey thanks for the reply, I’m trying to get in to see another dermatologist right now. it’s the only place that takes my insurance. I haven’t been to that place specifically because when I went there over problems with my scrotum (undisgnosed non specific itch for 8 years there), the doctor refused to even look at my issue!!! i’m broke at the moment so I can’t pay out of pocket for anybody else but I’m going to at least go see a doctor there for a second look. and yes my towels that I use to dry with our washed in the same hypoallergenic detergent.

      i’m honestly scared that whatever it is on my scrotum I’ve had for so long has somehow finally spread to my penis.

      i’m considering being circumcised in the near future because I’ve read about a few cases of particarly troubling balantitis that were resolved after the foreskin is removed. i’ve heard it’s very painful but what I deal with day-to-day is as well, and I’m very scared that it could make things worse or even just not make it better and then now my skin is exposed to even more things.

      but yeah i I am just constantly researching every day now trying to figure out if it looks like dermatitis, in my opinion it doesnt seem to.

      just out of curiosity what do you think looks more concerning about the third photo ? is that one where you can see the redness under my foreskin?

      I would not get circumcised with the belief that these issues are going to go away - you have a wider dermatological issue - that needs to be resolved, rather than removing a large section of your penile skin so that potentially the symptoms may no longer be present in that specific area. They may occur after the circumcision.

      Unfortunately, photos can be rather misleading, what you see in front of you is not always viewed in the same manner by anyone viewing the same symptoms in photographic images.

      But the 3rd photo seemed to be showing quite significant skin irritation

      i’m just not sure what to do because I can’t figure out what I’m having dermatitis from. i’ve tried for months now to dry properly and be really careful with that area in hopes of the redness and irritation leaving, and each topical medicine does nothing if not irritate it worse. They tell me it could be some form of contact or allergic dermatitis but I don’t know if I’ll ever figure it out on my own

      So you need to go back to simple basics: are you currently using a topical medication on your penile skin?

      If you are STOP!

      Do you have skin issues anywhere else on your body?

      Can we get this right on the detergent you are using on your towels and clothing, particularly your underwear, ANY sports shorts or casual shorts you may wear for activity or just lounging around the house.

      These should ONLY be washed in Liquid Soap Flakes - when you wash ANY of these ONLY use the Liquid Soap Flakes, you MUST NOT place ~ANY OTHER additive in with your washing..

      Even if you have to go to the extent where you but some new towels that ONLY YOU USE to dry your penis with - these too should ONLY be washed in the Liquid Soap Flakes.

      The next may be really difficult, but if you want to rid yourself this may be a critical element: no sex or masturbation for at least 5 to 7 months. If you see a Genital Skin Disorders specialist in that time and they say it is okay to resume sexual function then that would be fine.

      The FUNDAMENTAL with any penile skin situation that has developed and stayed in place in the way in which yours has needs to have two essential criteria for change:

      • whatever is, or the combination of things that are causing the irritation need to be no longer in place
      • for healing to take place in your circumstance the, or all sources of irritation need to be removed from the equation for at least 5 or potentially up to 9 months for full healing of damaged penile skin to recover

        .

        Simply, unless you put comprehensive measures in place of this magnitude and do NOT vary from this for an extended period of months I would say your penile skin issues will remain.

      I have similarly resistant, long-term irritation on my scrotum as well and I have for the last eight years. just itching, rarely it burns.

      is there a certain type of liquid soap flakes that you’re talking about ? and I would love to find a genital skin specialist but I don’t think we have them around here based on my research. Roger that on the towels and such, I’m just worried that I’m allergic to my clothing or something. i dont live alone so idk what to do there. I cant just go around naked..

      And yeah at this point i'm not expecting it to go away

      i cant be sure im not getting irritated by my clothes or something. I cant handle another 5- 9 months of the itching and burning, ive already lost my job and i'm going insane. cant talk to girls, nothing. there’s nobody in Kentucky besides regular dermatologists and they won’t do anything. thanks for the advice Wee, i jisy feel defeated already. I dont know how it could be constant pain and itching all day if its just an irritant unless its my clothes or something. I shower usually every other day and use lukewarm water, no products of any sort. Should i try using vaseline as a barrier for urine (which really burns)?

      its been even more red and cracked looking, i just wish i knew if i was doing the right thing

      I smoke marijuana maybe a couple times a week? went a few months without and didnt notice a difference. never had any sort of reaction elsewhere on my body. Do you think that smoking could somehow be irritating me? seems unlikely given the specificity of the area but at this point nothing seems crazy lol.

      Large Supermarkets will have Liquid Soap Flakes in their own brand names - go to where the soap powders and clothes wash treatments are and ask a member of staff if necessary.

      A barrier is the last thing damaged penile skin needs. Simply wash with water after every time you urinate. If you cannot wash use clean paper tissue or toilet tissue to remove any urine residue, then wash as soon as you can

      I would be extremely surprised if there is not a genital disorders specialist within the area of Kentucky.

      You need to find out who all of the professional associations are for dermatologists are in the States and phone then all if necessary to find listings of these specialists

      For the pain element of what is going on a Urologist should be able to guide you on dealing with that - as I also suggested I would be very surprised if Urologists and Genital Health specialists are not based in the same facility as they are both dealing with genital health, or at least they should be able to guide you on finding such a specialist

      I would suggest you would need to abstain for around 8 or 9 months to see if there is a positive reaction - for something like what you smoke you will not get a relatively instant positive response

      As I say, the vast majority of genital health specialists treat women and girls as the vast majority with actual genital skin problems are female.

      A city the size of Kentucky will have a number of Gynaecological health facilities and hospital departments - these from time to time will need to use the specialist knowledge of a genital health skin specialist, and yes, often the problem is finding online listed details of these, but they will exist.

      I would start by trying to find the largest Gynaecological health facility in the Kentucky area and try phoning them. Often such facilities are allied to teaching hospitals where specialist doctors and others such as dermatologists gain all of their professional training - again it is finding where these are.

      No you are most unlikely to be allergic to the clothing itself, what it could be is if what you wash your clothing in has an irritant factor about it, this could be part of a combination of things maintaining the genital skin problem and irritation that you have.

      And yes I have known a harsh (usually these are the cheapest ones) washing powder to cause severe genital skin reactions
