Not sure which treatment is best for BPH, can u help?

I am 75 with slightly enlarged prostate (30gms), Post Void Residual Urine 70 ml with normal PSA. Urine culture and Urine Analysis ae also normal. I have Urge Incontinence due to Overactive Bladder. There is always a leakage of few drops before reaching the toilet. I am also suffering from Erectile Dysfunction. I was suggested 2 methods of treatment. (1) Take one tablet of Cialis 5mg daily for BPH and ED.  (2) Take Avodart 0.5 mg and Spasmolyt 20 mg daily in the morning and Xatral XL 10 mg daily at night.

I shall be grateful for advice on the best method of treatment.

    Hello there.  Sorry that you are having so many problems.  I look up some of your med and you should be care full with them.  I was on cialis for a while it never help with my bladder or anything else.  For ED you should be taking a pill that is at least 100 mg to help you. for your bladder Try to see if your doctor will give you Myrbetrig 10 MG  I was on that and it relax my bladder great to good.  I was only going once or twice a day. Treatments :  Try something less invasive first.  Uro-lift, PAE are a couple to look up there are a few more but start with these 2.  Also get off Avodart.  it causes a decrease in libido and impotence.  I hope you can fine something to help you  Good Luck  Ken.

    I don't like this regimen at all. For overactive bladder, I would try Myrbetriq first - it brings the least side effects by far. The others give you dry mouth and constipation. Cialis is the best solution for ED because you take it every day allowing sex to be spontaneous. I disgree strongly with Avordart - it often CAUSES impotence, so for someone with ED, its counter indicated - but also not needed. Your prostate is normal, not slightly enlarged and you don't mention any serious issues with BPH, so I don't see the need for Avodart - I had pretty bad BPH and refused to go near it even then. Alfusosin (Urixatral) won't hurt you and doesn't usually bring any side effects, but seems unecessary.

    I would start with Myrbetriq, see if that deals successfully with your urge inconinence and Cialis. If Cialis doesn't work, the other ED drugs might. And if Myrbetriq doesn't give you the help you're looking for, older drugs to treat the condition might.

    There is a high probablility that Myrbetriq will solve your urge issues, ED is less predicitable. But unless you have BPH symptoms you didn't mention, I wouldn't take any drugs to address it - especially Avodart, which can be a major crap show.

    No meds will help. They all have side effects which will make you feel worse. Get 3 urologic opinions as to which procedure is best for you.

      I respectfully disagree. He doesn't describe having any BPH symptoms and there are no surgeries for ED or overactive bladder. Meds probably will help and surgery surely won't

      There is a very effective surgery for ED, that is a penile implant. I have one, they are great!

      That is great, Neal!  I always wanted to communicate with someone with a PI.  Do you pump yours up when needed?  What kind is it?  How long have you had it?  I've forgotten all the research I've done on these products, so I've forgotten the appropriate questions.

    That is exactly what I take, shareefmm.  My prostate is at 40 g.  These drugs at least make my condition tolerable.  Good, bad, or indifferent, I'm using these drugs as a strategy to buy time.  Yes, there are procedures I could undergo right now.  However, if my symptoms don't worsen over time, I'll procrastinate those procedures for as long as possible.  Things seem to be constantly improving regarding the research of prostate disorders.  One final note. Check with your health care practitioner about a potential interaction between Cialis and any blood pressure medication you might be taking.  Also check about a potential interaction between Cialis and any form of nitroglycerin you might be taking.  Best of luck to you.

    I'm most pleased with my UroLift surgery.  You can google it up.  Had it a year and a half ago.  Now getting up to 7 hours of sleep at night.  4 and 6 hours at least nearly every night.  Check with your insurance first.  I wound up with a bill for $18K USD.  Appealed and they paid for all but $1,200.  Happy to pay that for the relief.  Still have a sex life.  72yo
