Ongoing chest pressure with occasion acute attacks

For a friend: 41 yr old male, working an active job so over 13K steps a day. Ongoing chest pressure localized at the heart started two months ago. Sometimes when at rest but usually when active, the chest pressure increases to pain. When it does, left arm numbness and tingling. Several EKG's, all normal, no troponin showing, stress test normal, Calcium CT very clear. But the pressure is still there and it was relieved by nitroglycerin. Doctors are saying there's no heart problem, but there is clearly something wrong. It's driving us crazy and scaring us. Anyone have any ideas with what it could be?

Any other asociales symptoms? Shortness of breath? Sweating? Light headed, dizzy? Fatigue? Burping? Abdominal pain? Pain in shoulder blades? Palpitations? Any hypertension or high cholesterol? What's his heart rate? Does his heart ever race? Any blood labs that were abnormal? Inflammatory markers? CRP? Does your friend believe it's his heart??

He gets hot when it's acute but doesn't sweat. He does get short of breath during attacks. The pain feels like squeezing, even when its bad. No pain in the shoulderblades. During one of the acute attacks he became confused putting movies in movie jackets. He couldn't figure out which to put in what jacket for a few minutes. Cholesterol was good. Not normally hypertensive (130ish/175ish)but gets over 200/100 during acute attacks. I'm told the electrolytes and troponin are normal. There's a strong family history of heart disease so that's what he thinks it is. As far as I know they haven't run inflammatory markers. No burping.
