Ongoing problem with penis and testicles

I'm 23, and for 8 years I've had a pretty intense itch on my scrotum. I have been to numerous doctors and specialists, all of which claim the skin looks normal to them. I was first given a streroid cream, because in the beginning I had a red rash in the creases of my thighs, and they were raw. They said probably jock itch. Well, the redness and irritation between my thighs went away, but ever since then i have this strange itching on my scrotum. sometimes it also feels like a mild burning, and (especially if touched or if i have clothes or a bed sheet against it) will give these sharp, pins and needles feeling similar to how nerve pain has been described to me.

So I have seen dermatologists and they’ve given me a few different types of creams, maybe I didn’t stay on some of them long enough to see a result but i was very angry and scared about it at that point in my life, so i would try the topicals a few days, and it always seemed to just itch and burn the same if not worse, but with a greasy feeling on top of it. I've had A&A tests, probably every bloodwork you can think of, but nothing ever comes up. I'll have to call and have the actual results mailed to he again as i misplaced them, but I have also been allergy tested a couple times. once for foods and once for contact allergens. I never eat the one food it says i'm allergic to (shrimp). and as far as the other test i remember being mild-moderately allergic to just about every dye, fragrance, etc. And I use a special detergent. Next step is i guess not wearing any underwear or clothes with dyes.

So fast forward to recent times, and i notice i'm having some sort of reaction on the glans of my penis, it gets all shiny and wrinkled almost dry looking, and sometimes itches. the urethra is red and irritated and burns slightly when i pee sometimes. it seems to be more prominent when i'm erect, and looks like somebody sandpapered my penis haha. Anyway i'm wondering if any of you guys have experience with this or know anything about it, i'm really struggling here and appreciate any advice. image

    Hi Morgan.

    I have the same problem on my glans and will be going to a dermatologist again here in August maybe she can figure it out i can give you an update when ive been there 😃

      Hey thanks for the reply man, good luck! Definitely let me know if you find anything out. I'm in Kentucky and the dermatologists here really haven't impressed me any haha

    Morgan, I can empathise that this is very frustrating for you to deal with. What I think may help with a different approach is if I use a rather simple analogy: firemen deal with situations that have gone wrong, they deal with the resultant symptoms of incidents that occur. You need to be ahead of this process, and 'stop fires' from happening in the first place, or in other words, the symptoms of what you have been dealing with for far too long.

    So, yes, when you say: " Next step is i guess not wearing any underwear or clothes with dyes " - could be an initial look in the right direction.

    Your attention would need to look at the broadest possible scope: so I would need to know whether you are an active person, perhaps you do sports, or perhaps gym workouts quite regularly, and in particular, perhaps your regular activities make you sweat regularly?

    You may alternatively be inactive, and the other potential is you have hygiene issues that are not being addressed for the specifics of your skin type.

    I would certainly want to know how often you shower, and what you regularly use? Soap, shower gel, any other body wash, do you wash your hair in the shower? Here, it is what is in contact with your skin on a regular basis that is the critical factor - simple things like soap and shower gel are too easily ignored, but can have a huge factor, irrespective of whether they are the individual source of the problem or not.

    So, what you were getting at when you mentioned underclothes is a good starting point. Get some white cotton, if style is important for you, you can get leading brands that still have white cotton, and whatever colour waist bands they have is not an essential.

    The essential is, you MUST switch to washing all of your clothes, bed sheets and towels with Liquid Soap Flakes - this replaces soap powder or other soap treatments you use in a mashing machine. No other additives such as fabric conditioner should be used either.

    If you answer the question I ask we can take the next step in trying to get to grips with what is going on.

      Suffering pretty much the exact same thing. It's been ongoing for near on 7 weeks. I've tried a few antifungal creams/tablets. But no results. I have to ask, is the condition much worse following sex/masturbation? Mine flared up really bad recently, absolute nightmare. I've been to the docs been put on antibiotics for two weeks now to see if they do anything, so far on day 4, nothing. I visited a sexual health clinic, waiting for some results from there but very unlikely, only been with the same partner for years.

      As Wee_Dugie mentioned perhaps cut back on the soaps down there? I have, but to no avail.

      Keep us updated if you find out anything would be good to know what relieves/worsens the condition

      hey I apologize for the late reply and I’m sorry you’re going through a similar ordeal I know it’s definitely affecting my confidence with the ladies. and absolutely on the worse with masturbation part. I have not had a sexual partner since the issue began or at least since it became noticeable. but the second I start to get hard it immediately gets that really wrinkled and cracked shiny look. Have only masturbated to completion a couple times since it started and it definitely seems to worsen at at least the friction or for the duration/slightly afterwards. Seems that much like when I urinate, the semen appears to make my urethra irritated as well as this dry, irritated appearance. And I had a similar situation where I was on two weeks of antibiotics recently but nothing helped. I absolutely use no soap down there just water, cool or lukewarm. Have been trying some virgin coconut oil applied it very scarce only once a d have been trying some virgin coconut oil applied it very scarcely once a day/ every other day after my shower but no luck on it helping yet.

      Hey mate,

      I've pretty much finished the antibiotics now. As mentioned previously, no change.

      I'm going to head back to the Drs Wednesday. I have some routine bloods booked in for the same day not that i expect they'll flag anything related to this.

      Had the results back from the sexual health clinic, as suspected, all clear.

      Really at a loss now given the antifungals and antibiotics haven't done anything. Much like yourself I'm only washing with water down there, no harsh products.

      I've tried coconut oil, didn't find it to work but best of luck with it.

      Keep me updated if you see any results or hear anything more, I'll do the same when I see the Doc.

      sorry to hear that nothing has helped you either man , this is rough. The last week I've really been down on myself about it worrying that it may not get better.. but i have to stay hopeful that it will. It cant be permanent, right?

      yeah no luck with the coconut oil here ether. I appreciate you keeping me updated. I will do the same if I see a dermatologist again.

      Sadly they usually just give an antibiotic or an antifungal cream and then when that doesn’t work they just kind of give up. Also feel free to direct message me on here anytime with questions or with any progress you’ve made/not made. Excuse my language but i know this s**t sucks.

      out of curiosity, each time you've visited a dermatologist have you not asked if there's a test they can do to determine the cause rather than just chuck creams at you. That's where I'm at now, i want to ask the Doc if there's some sort of swab/scraping they can take to determine if fungal/bacterial. I'm a massive hypochondriac and given conventional treatments aren't working I'm worried it's something like cancer 😕

      On my next visit thats the plan is to have them do a scraping or swab (at least swabbing the urethra). i have asked before but they always just think their creams are the answer lol. and I have actually had a punch biopsy done on my scrotum but to be honest I don’t trust the competency of that doctor to have really looked over the findings in their entirety, they told me there was a presence of white osteophytes or something? Said it suggests there has been inflammation in the area for an extended period. But then they did nothing to follow up on that. My testicles have been bothering me so bad with the itching, as well as the prickly sensations that these last few weeks it’s been a complete nightmare. After 7 years i'm afraid it may not be curable or that I will just never find the answer or diagnosis. but it also bothers me that the glans on the my head of my penis look so red and shiny and just irritated no matter what i try, i've been an anxious mess i'm not even employed at the moment because 1) i need the state insurance and 2) i'm so itchy i cant even wear jeans or khakis

      Hey buddy,

      So I've been back to the doctors today, was saw by a new doctor this time. He checked the situation over and said he can see what I'm referring to by redness. Apparently it doesn't look like anything nasty (which i suppose is good, - although it wasn't flared up).

      He said that I can't be referred to urology until I've at least tried a steroid cream. So I've been given a Canesten HC combo cream, I'm to apply this twice a day for a maximum of two weeks. If no joy then I can get a urology referral. Absolute nightmare! Just want it gone.

      Hey man hopefully the cream helps, I know its maddening Especially when the doctors won’t even test you or really tried to the maximum extent of their ability. I’m doing all right I guess. I was with a girl for the first time in months last night, after we finished the head of it just burns and feels really irritated. I don't know what to do, this is really a trying situation.

      Are you in the US man? I'm not really sure how your health-care system works but maybe its time for a biopsy on the glans?

      I'm not holding out hope for the cream haha but, yeah, fingers crossed. If not I'll be demanding a urology appointment next visit and then I want to go straight to biopsy just for own sanity at this point.

      But high five for the girl last night brother! don't let this get you down too much! Keep me posted and I'll do the same

      Yes US here, Kentucky specifically. yeah I'm going to suggest the biopsy to my doc, as well as the swabbing and another urine test. and thanks man, she's a really sweet girl and it just makes me feel bad that my dick is all burning and red and shiny looking, and i just pray it doesn't stay like this. I will certainly keep in contact, I also have e-mail and texting if correspondence would be easier that way.

      I'm a week into the steroid cream come tomorrow, no progress really, it just looks the same as usual provided I abstain from masturbation not to aggravate it. Two days ago i thought i had a break through when it looked pretty normal in the shower but it was back to looking bad again after a few hours.

      Email might be good if you want to give me yours, text might not be the best coming from the other side of the pond.

      its cool man i forgot to hit send all the time on stuff. Same here, and sorry to hear that. It’s not quite been a whole week for me yet but I’m on a antifungal cream twice a day and two antibiotic’s per day. No improvement yet.. honestly the urethra is even more swollen and red than before, stings pretty bad when i p**s, especially afterward. I'm hanging in here i just pray this gets better soon (and for you too, man). I haven't even been intimate a second time with this girl.. it gets so red and wrinkly and shiny looking and burns so bad after, it's beginning to get pretty disheartening. best of luck to you though

      Little update for you pal.

      The Canesten HC did not work. So, I returned to the doctors and this time saw a different doctor.....again. He didn't really see any issues. recommended I stop worrying and just apply some general moisturiser to the area such as E45. He did however prescribe a weeks worth of Flucanozole just because i asked about it potentially being a lingering yeast infection where creams alone haven't tackled it.

      Anyway, i applied E45 for a few days and the redness died down a fair bit, not to the point of being gone entirely but nowhere near as bad. I stupidly then thought I'd best apply the Daktarin antifungal cream again just incase, it looks awful again so now I'm wondering if the area has actually been aggravated this whole time by antifungal creams, yes, it existed before I started applying them but maybe i just jumped on the wring wagon from the start and its only helped it linger.

      Im switching back to just the E45 3 times a day. I'll keep you posted how it goes.

      hey man any luck? i was on the antifungal/anti yeast medicine right after the doxycyline (antibiotic), and i arill cant tell a difference. In appearance or feeling. It may actually be burning worse

      Hey Morgan,

      I'm not sure if the antifungal tablets have done much. Application of E45 seems to reduce the redness in the area, I can't say that I've gotten rid of it but it looks physically less aggravated most of the time now so it may be worth a shot.

      I'm going to keep at this regime a bit longer before returning to the Doctor. My logic is now, although it might not be sound, but if this was something awful like cancer it wouldn't vary in appearance from hour to hour/day to day.

      I'll update your further in a few days but also noted since using the E45 post mastubation aggravation/redness has died down a little too

      Hey buddy glad to hear that! E45 is a cream they prescribed or is it over the counter? mine has honestly been a little bit worse it gets terribly red and still has the cracked look to it, the urethra is very red and inflammed. Being sexually active bothers it way more than urinating. it stings slightly when i pee sometimes, but every time ive masturbated or been with this girl, i can tell its aggravated during the encounter but afterwards, when i finish, it looks even worse, and burns like hell. It's driving me crazy. I’m starting to wonder if I’ll ever be able to be sexually active again without having such discomfort and pain

      E45 is just a standard OTC moisturising cream to help with conditions such as Eczema, Dermatitis, Psoriasis and whatnot.

      I suggest giving it a go for a few days if you can. Like i said, mines better, not 100% cured but there are times where it looks normal now after application so that's something atleast 😂 and definitely the "cooldown" time for redness after masturbation/sex is better.

      I'll keep at it, but try to abstain from sex for the next two weeks to see if i can be completely rid of it.
