Options for either a HoLEP or PAE

Guys, at present  an tethered to a catheter and am mighty sick of it. reason for this is my template biopsy early Feb (32 cores taken) and large (> 100cc prstate) seems to have tipped it over the edge. Two TWOC failed and for sure I don't want to go through that experience again. Not only that but catherisation for me involes a cystoscope so it is not straightforward to re-insert.

Urology offered me the HoLEp which from what I read is a 'good' option. Privately (here in the UK) I can have it done on Sunday! However, i am investigating the PAE option and have as yet to meet with any specialists though that is on track next week but no idea as to how long it will take to reach a conclusion. I can also do that privately in 3 - 4 weeks. The costs for those interested is around 5.5k UKP. I amprepared to pay for a quck resolution to this drama.

So, just wondering about opinions here please?

    I know the feeling.  I had to wear a catheter, for around 3 months, if I remember correctly. Now I have to self-cath, and it's the greatest!

    PAE is a less evasive procedure.  It carry less side effects then Holep.  You can all way try PAE and if the don't work go for something else.  PAE you have a better chance of not getting retro.  Holep is a giving and you may have some leaking And sometime you end up having a turp because they have to go in to fix the mess from the holep  Look at your pros and cons.  Don't just take the doctors word  Ken

    Tissues taken during HoLEP will be examined by pathologist - probably something worth considering seeing that you had a bunch of biopsies.

    In my case, positive results from HoLEP were within a week. Actually, I was able to urinate freely 24h after surgery, but expect healing process take, roughly, two to three weeks.

    I knew of PAE, but at that point I just wanted to get this s__t done as soon as possible. Also, my insurance covered HoLEP, but not PAE. In hindsight, HoLEP might've been the more viable option since I had both the median lobe and bladder stone.

      thanks for that and I agree I was tempted to go on next Sunday but I have an apptment with a hospital that does PAE and I would like to find out how long that would take to get done. My next best choice is hoLEP on 23rd April, so not too far away and I appreciate the feedback on your experience that does not sound too bad.

      Hello.  Are you sure that Holep is your best with the side effect that can happen. No doctor knows for sure what is best for some men because we are all different.  They really don't know if it will work until it done. And if it does not work they fined something else to blame.  While you have time cancel the procedure on April 23rd.  Look into the PAE procedure I think you may find that one to better for you.You have the choise to pick what ever you like but there is no going back if it goes wrong.  ( Did your doctor tell you sometimes you have to have a turp to clean up the mess from the holep if it does not work. wink

    Guys hospital is currently doing a trial of PAE and you should be able to get a referral on the NHS to get this done fairly quickly. Probably worth a try
