PAE and other procedures

Hi everyone,

I am looking at options for BPH and so far it seems to me that Urolift seems to be the best option to preserve full sexual functions .

However, ive just been reading about PAE , has anyone had this done ? I don't fancy the actual procedure which seems to me could be very painful.

Any input appreciated

    It isn't painful, though at times I felt a little uncomfortable. It is always going to depend to a large extent on the skill of the Radiologist, so look into how often they have performed it. It is important from your point of view to make sure your condition is conducive to the treatment. It is becoming clear that BPH covers a lot of symptoms that not every treatment is suited to. PAE's advantage over any other intervention is it doesn't preclude or affect any alternative. It therefore is the first option to consider for your issues before heading further up the intervention options.

    Hi d,

    I had a PAE with Dr Bagla in VA. He uses twilight sedation, so I was completely out during the procedure. Very little pain afterwards except for 2 or 3 days of burning during urination. PAE shouldn't affect sexual function.

    I had a PAE in July of 2015 at the St. Louis Un hospital in St. Louis Mo. It did nothing for my BPH. The IR was only able to get to one side, and that side shrank in the outer regions, not the core. I just had a urolift (I'm 10 days out). The recovery is more painful, so far, than the PAE, but I'm hopeful that it will help. Can't comment on sex yet, since it is off the table for at least another week.

    The PAE was very expensive - $7000 from me and $37,000 from my insurance - for nothing.

      The only positive I can report so far is that I have not had to self-cath except for the first night after the procedure. I caused myself a problem by breaking the "lifting" restriction too soon. After 11 days, I was feeling pretty good, and picked up an item that was about 50 pounds. Within 30 minutes, I was having some serious pain. It has now been 3 days since I did that, and the pain is just now subsiding. No blood, but it felt like a large cocklebur was stuck at the site of the urolift. It has now been two weeks since the procedure - flow hasn't changed much, but starting and stopping is better than before the procedure.

      I can't comment on the sex, since I have followed that restriction. I was told not to attempt sex for 4 weeks. Some people report leakage - I haven't had that happen.

      Good morning.

      Yes they tell you no sex for 4 weeks because they want the clips to a here to the prostate. But I would maybe wait a few more weeks because of what you did. Lifting up that 50 pounds may have move the clips but being you did not have any bleeding I think you were lucky.

      Just take it easy and relax. Drink a lot of water and stay away fro salt.

    I'm not going to push the Urolift But that is the procedure that I picked. For the reason you mentioned . I did not want a problem with sex or l did not want to lose my ejaculation. Some doctor do not feel that we need it as we get older because we are not having kids. It is not there choice.

    PAE is not done by a Urologist it is done by a Radiologist. It has work for many men. How bis is your prostate. They say it works better if the prostate is 50 cc or more because the arteries are bigger. They say that you will not lose your ejaculation but some have found out the the volume of the fluid is less and sometimes stops

    There are some get articles on PAE that you may want to read. But it is up to you what side effects you can live with. Take careKen

      Thanks mate , at this moment in time im not how big my prostate is . ive always been told its normal size but i have a severe blockage . I have a cystoscopy booked for late November Just trying to get a handle on all the options as im not keen to continue self cathing . i want to be fully informed so im not led down any path by the urologist to suit his own agenda. I seem to recall some time back that you ( apologies if it was not you) shared a list of all options for BpH on this forum . How can i rediscover that very helpful info ?

    I had Urolift 3 weeks ago. Very painful procedure and painful for first week after. At 3 weeks pain is mostly gone but I swear I can feel the clips on occassion if I get constipated it really hurts. No improvement in flow but about where I was pre-procedure. I have started getting up less each night and I feel like I can empty my bladder a little better. Still hoping for more improvement going forward.

      It will get better but I would never have it again if I had to have it done in the office. I had mine done in a surgery center. Drink a lot of water and relax. Time heals all Ken

    Hi! I just had PAE from Dr. Bagla in Virginia. It was a completely painless procedure. I have resumed normal sex, though for the first few days after the procedure I had a little burn on ejaculation. I am quite satisfied but am looking for the one month date to see how much I have improved in the urinary urgency and frequency scale. I am not quite as frequent as I was but my urinary urgency seems to not be changed much. Each time I pee I notice a little more flow and a little less nighttime up and down. Good luck!

      I am happy that all is going good but I would have held out a little longer for the sex but to each his own. Give yourself some time to heal and have fun

      Hi Ken; The Uro I currently see has done over 100 rezum treatments in which honestly, I'm not wild about. RE for me is not a big deal. I'm basically celibate due to my religious beliefs. I understand RE is a bigger deal for other patients - all good. My issue with Rezum stems from the fact that they just go in there and blast away. To me, it seems like a longer delayed Turp. I've considered Turp procedures but the possibility of strictures from that bother me. GL, same, so I'm considering the urolift. I know you have a stricture yourself. How has that affected your overall symptoms? I'm in The Vegas area, so we have Uros who do Urolift here. FLA is something I'd consider. PAE - not keen on that.
