Pain in perineum, 9 years, following TURP

Nine years ago I had a TURP, about a year later I had a second TURP by a different Urologist, ( that seemed to know what he was doing and was referred to me by a PA at the same medical facility.

He preformed a "green light laser TURP, (different than the first).

This was done as a result of my complaint that the pain in my perineum area never completely went away following the first TURP.

That second one did not really take care of the stupid pain that occurred at urination and defecation, ( and even when I pass gas). In addition to this pain which is typical of pain that one is told to expect for a couple of weeks following the procedure, I also had some very weird things occur. For example when I heard water running that same pain occurred, when I filled my car with gas, same thing, (sound of liquid).

Now I realize that some of that is really weird, but it was real for many years.

As my Urologist would try to continue to get to the reason finally following a cystoscope examination the pain went away for a couple of days, (maybe a week), but it came back and continues.

I was ready to try hypnosis and/or acupuncture by finally my GP suggested that maybe there was a nerve that was "unhappy". I had been suggesting that for years.

So, she has suggested "pain management" and that's my next step.

I'm just wondering if anyone else has had anything like this.

    You are seeing doctors that don't seem to know their practice and are going far afield into different practices. Get some more opinions.

    John: I can say that I have something similar to this but it is not a pain. It is an urge. If I get the urge to empty my bladder, as I walk to the bathroom thinking about it, the urge gets stronger until I wonder if I am going to make it on time. If water runs or I hear the sound of liquid flowing, the same urge comes upon me. I have come to the conclusion that is a sort of "mind over bladder" thing. If I get the urge and I decide I will hold it for fifteen minutes until I finish watching a show or doing something outside, there is no increased urge. But the minute I head toward the bathroom, knowing that I am going to empty my bladder, the stronger urge comes upon me. I have now learned that since it is a mind over bladder thing, I can keep the urge to a minimum as I walk to the bathroom and I never have an accident as long as I tell myself I will wait until I get the bathroom before I empty my bladder. I think the brain/mind has a lot to do with my particular issue.

    Have been on this forum for many years and have never heard of a "green light laser TURP" - sounds like a basic green light laser procedure. A GLL procedure uses a laser to reduce the prostate tissue. A TURP uses a cutting loop and/or plasma buttom to reduce the prostate. The older style TURP (which is probably what you had years ago) was/is called monopolar TURP, and the newer types are bipolar and plasma button. The monopolar TURP is being phased out.

    Also, I have never heard of anyone having the type of pain you are experiencing, so you are going to have to get a second, or third opinion from another urologist. Maybe someone here can think of something. A TURP goes up the urethra and not through the perineum, so this one is a bit of a mystery.

    Very sorry to hear about your pain issue, and i hope that you find a solution for it. Also would like to ask you how well the urethra will tolerate a 2nd TURP is the first one doesn't work as well as expected?

    John: I too am wondering how well the urethra can tolerate a second TURP. The TULSA Pro procedure actually cools the urethra and the rectum to eliminate damage to those tissues while the procedure is being performed. Perhaps you could look further into TULSA and consult with a doctor that performs it in your area.

    Hi John,

    Yes I had a TURP done by a knife wielding surgeon in 1996. But no pain after just great relief at being able to urinate freely again. But the biopsy found traces of Cancer and I had a full radical in November the same year. Since then I have ha a lot of discomfort. Then in 2002 I had to have 37 doses of RT as the cancer came back and the discomfort worsened as I now get regular pain in that area due to damage caused to surrounding tissue by RT. I also need to go when I hear water running but that is a very common thing. When horses had urinary problems and couldn't urinate the vets used to stand by them and whistle gently and run water into a bucket. It worked. The human body reacts in odd ways to sights and sounds. Watching someone else yawn, increased saliva when watching someone suck a lemon, clearing the throat when someone else does it. I wouldn't worry too much about it. It doesn't sound like you have a serious problem.
