Painful urination

I have been feeling burning sensation along with white liquid like layer on the head of the penis. This often happens when i go for stool. This is not so frequently it happens with me like once or twice in a weak .and it remains for few hours and then pain and the sensation of burning goes off.

i did urine culture twice. first time there is bacteria and i used many antibiotics. But as soon as the medicines runs out, it's back .then i did urine flow rate test, there is no problem with speed.i want to urinate often and there's lot of urine.This is about two years now(on and off)

what is this? I'm 20 years old boy.
