Penile Rash/Balanitis Not Responding to Treatment

This all began when it started to burn while I urinated. I went to the doctors because I thought it was a UTI, but the test came back negative. Got tested for STD's, the tests also came back negative.

Ok no big deal, I thought. Maybe this will go away on its own. I then started developing a rash on the tip of the penis and along the base that burned post urination and during ejaculation.

I did some research online and thought it was probably a yeast infection. Bought some Canesten and tried applying it twice a day for a week. I also took the pill (Fluconazole). This resulted in no change in the symptoms.

Went back to the doc, who then prescribed Fucidin H cream (Hydrocortisone 1% Fusidic Acid 2%). I rubbed it on twice a day for a week, no change.

Went to a Urologist who did an examination (but no further tests) and told me I could get circumcised, but apart from that offered no further solutions. Told me it could be prostatitis.

I then went to a Dermatologist, who looked at it and prescribed me Loprox anti-fungal cream with hydrocortisone (1%) told me to use it twice daily on the area for one month. I then switched to just Loprox because it appeared as though hydrocortisone was making the irritation worse. The Dermatologist told me to go see the urologist again if symptons don't improve.

I'm still applying Loprox two weeks into treatment, and the changes are minimal if any. The rash goes down at the tip and expands at the base, almost triangular in shape. There are small scales (or maybe cracks) in the skin, and the area is red, almost as if inflamed. I have no discharge from the penis. Any kind of friction (i.e. masturbation) makes the redness and burning much, much worse. I have been abstaining from that for now and only washing the area with warm water. I have thrown out all my heavily scented soaps.

I am in the process of attempting to get an appointment with another urologist in order to get a second opinion. Topical creams don't seem to be doing much, but I am going to keep putting Loprox on it for now. I feel like the Docs have been kind of brushing me off/don't want or don't know how to deal with balanitis. Nothing I am really doing is working, so I'm wondering if anyone else has any solutions.

    I am very thankful I have found this forum.

    I've been dealing with same/similar symptoms as the rest (dry glans on the tip of my penis very irritated) this currently has gone on now for 6 months.

    I was in a long term relationship for 6 years never had this problem, left her though because I felt she cheated on me.

    I asked my Dr. to give me a blood test and urine test to check for STI's but he forgot to check for Chlaymdia!

    3 months later I met a new girl, we had ended up having unprotected sex,.

    And 3 months into the relationship she kept thinking she was having yeast infection problems, but ended up having Chlaymdia she then told me she hadn't been check since 2018,which I find disturbing since she had multiple partners in that time. When I did the urine test for Chlaymdia it came back negative, but nonetheless was over prescribed antibiotics, (doxycycline for 10 days, and 4 azithromycin) I should've just took the 4 azithromycin only. During that time I had my first vaccination shot...

    Soon after I kept having a red tip of my penis that felt very irritated and sensitive whenever I moved.

    I did the standard get checked for all STI's they all came back negative. I have had 4 different creams, weeks of Fluconazole no change. I thought it was candida infection due to over prescribed antibiotics as I was unable to eat wheat, or gluten with out brain fog, but now I believe that was possibly caused by stress due to worry of my penis and new girlfriend possibly cheating on me.

    I have gone to 3 different Doctors and a urologist who looked at it less then a minute and said looks healthy to me, I said, " but he really hurts sometimes and is very irritated!" He told me to just put on cream for diaper rash!

    Currently just very frustrated with the whole situation it has literally gave me insomnia and stress issues that I know don't help with the current situation.

    I personally feel like it is some sort of new HPV I have gotten from the new girl friend.

    Thank you to all that have been posting here as I don't feel so alone in this battle.

      Not recently no. That was one test in March that my Dr did test me for and I was negative. But I dont think its HSV as it is just very red at the tip of the penis and no blisters. I think the Urologist wouldve been able to tell if I had HSV from the look of it.

      agreed mate, just i have tested for HSV1 by blood and never knew i had it so trying to see any similarities

    i have just heard back from the clinic and the small cuts I get at the top of my bum cracked tested negative by swab test for herpes.

    Absolutley no idea what is going on with down there, it aint right though. Its like urine and semen get caught in the urethra and doesnt fully empty.

    Oh man I definitely feel as if I'm headed down the same path as most people on this thread, that being doctors and specialists just don't want to know, or simply DONT know. I pretty much have the exact same symptoms everybody is talking about which started 2 or 3 months ago when I had a slight cut on my frenulum which bled quite a fair bit. Ever since then I've had an horrendous few months with my foreskin and head of my penis being chronically inflamed. Tried the usual: Anti-fungal for 2 weeks, Hydrocortisone, flucloxacillin for bacteria that was confirmed through a swab which seemed to improve symptoms slightly but never got rid of them, which baffles me, the anti-fungal and steroid cream did nothing but aggravate it even more. I'm currently waiting to see a urologist but reading this thread really does not fill me with any sort of optimism.

    The only differences I have to most people on this thread is that I have never had sex so it cannot possibly be anything related to that but it seems most peoples symptoms started after having unprotected sex. Aswell as the fact that I have a mild case of phimosis, but it is definitely an issue, which my GP has advised I will most likely receive a circumcision for as I've done stretching exercises in the past which yielded results, but I wouldn't possibly dare try to stretch my foreskin in the condition its currently in.

    I will most likely get the circumcision and am just praying to god it fixes whatever is so messed up down there.

    I am joining this topic after having the issue for almost 2 months now. Thanks for all your posts! Here's my contribution.

    Redness and slight feeling of irritation around urethral opening (photos after hot showering and one of the creams described below so the surface is more "shiny"). Have seen a GP, an urologist and a dermatologist (also STD specialist). All have said that it doesn't look too bad at all but the symptoms have been not so visible at the appointments due to less blood circulation etc. (as many have said here as well). So the doctors seem to think it's nothing to worry about but as with most of you, I find it hard not to worry.

    I have tried different creams:

    • Pimafucort (cortisone and fungal medicine) - did not do anything
    • Fucicort (somewhat like previous but stronger) - made the glans generally drier, did not help the redness/irritation
    • Hydrocortisone 1% - has not helped much, maybe a little, now trying it combined with a moisturizing lotion as someone here earlier suggested as having worked for them
    • Canesten (fungal med) - no help

    Tested for chlamydia, gonorrhea and HIV, negative. Tested my urine for bacteria, glucose, etc, all clear.

    Now will try the hydrocortisone + moisturizer for a week or so as well as not touching, not squeezing the last drips (after peeing), not masturbating and especially trying not to think about it for a while. Hope this will help, of course it seems that it's unlikely as you guys must have tried all these. 😃

      My penis looks similar to yours but very mild if compared.

      After the hot shower I always have some dryness problems.

      I've been to 12 doctors:

      • 1 said its lichen sclerosus and after some months changed his mind saying its nothing
      • other 11 doctors said its nothing

      I've been to a lichen sclerosus expert too from the specialized hospital center and said its not lichen, but dont know what's the cause.

      I've made all tests possible, except for biopsy.

      The most of doctors said that the only thing I have to do was LEAVING IT ALONE.

      It worked.

      But I'm still strange and have strange sensations after ejaculations.

      I used a lot of products:

      • Antibiotic pills
      • Anthisamine pills
      • Steroid pills
      • Antifungal pills
      • Antibiotic cream
      • Antifungal cream
      • Topical Steroid cream (made everything worse)
      • Emollient creams
      • PEG creams

      Honestly I'm slight better but dont know what is actually helping.

      Doctors are not helping at all.

      I have had the same experience though no pills as medication earlier, just creams.

      Meaning that the best results until now I have gotten by leaving the thing alone, wearing loose fitting underwear and avoiding any chemicals, lotions, creams. The area went pretty much normal in maybe 2 weeks of intensive "leaving-it-alone". Some dryness though and the skin area around the urethra seemed to be not so elastic. However, time came to not leave it alone, meaning sex with a condom (with some chemicals) which pretty much brought me back to the same redness.

      I have no idea what the cause is or why it appeared now and only around the urethra but I suppose it is a combination of my bodily fluids and chemicals that is causing the irritation. Possibly some preservatives in lotions and same or similar substances in condoms?

      Anyway I am sort of happy to know that I can make the symptoms go away or at least lessen even though it means abstaining from sex/masturbation for some time.

      hi you mean it´s gone for you? you stop evrything and its gone? im dealing with this since september 2021. so you just stop the creams and the pills and its gone?

    If helpful to anyone currently going through this. I had similar issues - tried flucinzol pill, tried Daktacort for fungus, it ultimately turned out to be bacterial and Fucidin cream seems to be helping a lot.

    Ask your doctor to do a bacteria swab, try Fucidin for two weeks but also have a full STI screening (ask for the additional ghonnorea swab as it isn't standard) just to rule those symptoms out too.

      I don't know exactly the bacteria. Swab is from the glans with foreskin rolled back.

      I'm rather curious too. My issue seems to re-occur with friction I've noticed. Kind of at my wits end with all of this, whatever it may or may not be. 😕

      What Bacteria have you find on your penis?

      You made a skin swab or urethral swab?

      I have to try Fucidin cream too, please what bacteria have you find?

      He did a glans swab. I've done 2 uretheral swabs. Found nothing.

      Maybe a glans swab is next up.
