Penile Rash/Balanitis Not Responding to Treatment

This all began when it started to burn while I urinated. I went to the doctors because I thought it was a UTI, but the test came back negative. Got tested for STD's, the tests also came back negative.

Ok no big deal, I thought. Maybe this will go away on its own. I then started developing a rash on the tip of the penis and along the base that burned post urination and during ejaculation.

I did some research online and thought it was probably a yeast infection. Bought some Canesten and tried applying it twice a day for a week. I also took the pill (Fluconazole). This resulted in no change in the symptoms.

Went back to the doc, who then prescribed Fucidin H cream (Hydrocortisone 1% Fusidic Acid 2%). I rubbed it on twice a day for a week, no change.

Went to a Urologist who did an examination (but no further tests) and told me I could get circumcised, but apart from that offered no further solutions. Told me it could be prostatitis.

I then went to a Dermatologist, who looked at it and prescribed me Loprox anti-fungal cream with hydrocortisone (1%) told me to use it twice daily on the area for one month. I then switched to just Loprox because it appeared as though hydrocortisone was making the irritation worse. The Dermatologist told me to go see the urologist again if symptons don't improve.

I'm still applying Loprox two weeks into treatment, and the changes are minimal if any. The rash goes down at the tip and expands at the base, almost triangular in shape. There are small scales (or maybe cracks) in the skin, and the area is red, almost as if inflamed. I have no discharge from the penis. Any kind of friction (i.e. masturbation) makes the redness and burning much, much worse. I have been abstaining from that for now and only washing the area with warm water. I have thrown out all my heavily scented soaps.

I am in the process of attempting to get an appointment with another urologist in order to get a second opinion. Topical creams don't seem to be doing much, but I am going to keep putting Loprox on it for now. I feel like the Docs have been kind of brushing me off/don't want or don't know how to deal with balanitis. Nothing I am really doing is working, so I'm wondering if anyone else has any solutions.

    I'm following this post for many months now, since I am suffering for same condition as you guys are, since March of this year. Been going through dermatologists and urologists without any luck. I have had antibiotic treatment, anti-fungal treatment, and all kind of other redness relieving cremes. Nothing seems to be working. STD test are all negative. I really hope someone will come here with a solution, or at least something that reliefs the pain and discomfort. Wish you guys all the best, and please post a reply if someone finds what may be wrong.

      I post extensively on this issue, many of my responses in this forum deal with similar issues to what you briefly detail here.

      Through personal experience and following what other males on here experience the only way to return the penile skin to its natural and this means not applying anything at all to the skin, exclude soap and all forms of body wash - some males find that they even have to go to the extent of changing what their underwear and sports shorts are washed in and switch to using Liquid Soap Flakes for clothing in contact with the penis, and potentially towels as well.

      Yeah, since I am not so sexually active guy, I remember it starting after sex which for me was first in 2 years. I experience same problems even months after event. What user @Wee_Dugie does not seem to understand is that I have tried all of these techniques, which were proposed by my dermatologist, but they just fail miserably.

    hello Jack,

    i have been having the exact same issues for over a year now. our experience is almost identical to a tee, negative tests, multiple doctors visits and no change in symptoms to date. was recently prescibed hydrocortisone cream however this has only made things worse. any update on your condition?

      join the "men suffering from cpps" group in face book and you'll see how common this symptom is to cpps. I have cpps and I have this symptom myself

      .... and search for dermatologist in that group and you'll see how that's a symptom of cpps. cpps is caused by anxiety causing involuntary pelvic floor muscle spasm

    Hey guys I developed a similar problem last year. no burning no stinging though for the most part, redness, itching and one cut appeared at its worst point. I went to two urologists and tried a variety of creams as well as an internal pill that women use for yeast infections. Nothing worked. I finally gave in and got a circumcision 8 months ago and thought it went away but slowly came back after repeat masterbation. I do have prostatitis I've had it for about 5 years but in my case it's not related to that. I just dont think so. Prostatitis usually involves frequent urination as well as burning among other things. I've had that under control. The one thing that really seems to be working but i'm not going to say that it's cured it yet is Lotrimin Ultra. Yes, an over the counter athletes foot cream. Someone suggested that on Amazon of all places. It has almost completely cleared it up in two weeks, better than anything else I've used or tried, but it's still there a little bit so I’m going to keep at it for another week. At this point it's really only on one side now as it was completely covering the entire circumference of where my foreskin used to be at one point.

    So it seems to be working, for me it's these 3 things:

    1.Clean and dry at all times. 100% cotton boxer shorts. Hair dryer after a shower.

    2.Lotrimin Ultra

    3.Take it easy with sex\masterbation. Refrain to only once or twice a week and NONE for the first two weeks of using the cream.

    I had the same exact thing, and nothing was making it better. Cortizone creams made it worse, and I took all these antifungals (and I doubt it's fungal) and I was running out of ideas.

    Solution:

    Do not use anything on it except water and try to avoid masturbation.

    And the WONDER drug: Mupirocin

    Go to your Dr. have him prescribe this and apply it 2 - 3 times a day. Within a week I was SO much better, and now 2 weeks later, all gone.

      Hey there klas-iris. Can you expand on this a bit? Was it a bacterial infection on the skin of the glands? Did you have any other symptoms (urinary issues, pain with ejaculation)? I haven't tried this particular antibiotic ointment, but I have tried Flucidin H, and it did not work. Did you try Flucindin as well?

    Also have the same. For a year. STD negative, creams not working, etc. etc. All the same as all of you.

    Mine actually started a year ago after unprotected ORAL sex during a one night stand. Normal sex was protected.

    2-3 days later I got small blisters on my dick head that went away after a few days. Also got rashes on the inside of my elbows. Rash also disappeared after a few days. I did all the tests at that point in time and got antibiotics "just in case". Because I'm married and don't want to expose my wife to my stupid stunt.

    Anyway, all came back negative and the rashes cleared so all seemed OK and had sex with the wife again. Then she got the same rashes on her arms and a fungal infection. To my big surprise. She had that once or twice before so did not link that back to me. She got anti fungal cream but I guess the infection was quite bad. Did not want to go away until she had three treatments (sets of pills). Anyway, after that my wife's complaints all went away but mine stayed. Same as all of you...

    Just wanted to add the "extra" information about the rashes and the effect on my wife. Maybe someone recognises it.

    As for cure; no clue... Tried everything like most of you. Nothing helps.

      yes and no...

      I have herpes for almost 20 years. So does my wife. When I got it I also got blisters on my penis. A few 2mm blisters filled with fluid. I remember well. I got 2-3 outbreaks the first year with blisters on my penis. But ever since no more blisters. Still have an outbreak once or twice a year but believe it or not I get an itchy red area with blisters on my butt. My penis is fine. This whole balanitis thing is new and different. The blisters I got now are way smaller than the herpes ones from long time ago. also no fluid inside. Just little red dots. But only once when I contracted this.

      So yes, I do have Herpes but the current issue is not caused by it.

      I bet you it's CPPS / tension in the pelvic floor miscles caused by your anxiety after the exposure which got you into a vicious cycle of anxiety - pelvic floor muscle tension - anxiety. I also have a red rash in the penis head that comes and goes, after a risky oral sex.

      Stretching the pelvic floor is the only known cure to CPPS.

      There's a private CPPS group in Face Book with thousands of members and tips on how to cure it. It's called something like "Men Suffering from CPPS". If you search the group you'll learn how many people also had a red rash and other symptoms

      But now you're assuming anxiety. Not the case. Totally relaxed about it. I've come to terms with the situation. It doesn't bother me too much and apparently there's nothing that can be done. So I'll follow the forum topics just in case but other than that I'm cool with it... No anxiety 🙂

      From what I know, if one's immune system weakens, the herpes virus can take advantage and develop itself on other places and/or worsen one's condition. This is what obviously happens when aging - check immunosenescence on Google. Don't wanna sound too negative though. Why not trying a suppressive therapy and see if things get better after 4 weeks ? 500 mg valaciclovir daily. Only if your renal function is OK. Sorry for my english, it's not my native language.
