Penile rash mystery

Hey everyone.

So I have had this mysterious rash that has constantly changed in appearance for about 2 months now.

At first it was suspected to be thrush, but when it didnt respond to anti fungals I had a swab test for bacterial and fungal components which both came back negative. So I went to a urologist who said it could be lichen sclerosus as I have a tight foreskin as well.. but I have had that for pretty much all my life. Its not a new thing. My dermatologist wasn't totally convinced on the LS diagnosis and he even said it could be a 'general dermatose of the penis'.. which felt like a cop out to me.

I was given dermovate a steroid cream to take down the rash but it only makes it less blotchy and doesnt actually make it that much better at all. 

I'll post the pictures in chronological order to you can see the changes in appearance that have occurred. Has anyone experienced something similar? Also, anyone with Lichen Sclerosus, does this pattern or appearance look familiar? From photos I've seen and descriptions I have heard it feels different.

I should probably note that I did test positive for HPV 44 recently, incase anyone deems that relevant.

    Hi, you are saying you have had LS pretty much all your life: has this actually been diagnosed with a Biopsy?

    Describe how often you are using the Dermovate, the frequency of application and the amount of time you take to rub it in?

    I have Lichen Sclerosus (LS / BXO in Males) and while each sufferer has their own experience of symptoms, what you describe and the images you have look very familiar to LS.

    Do you limit your diet to any particular foods?  Are you taking any Vitamin and/or Mineral Supplements?

    Do you apply any Lubes, regularly use condoms, apply Baby Oil, Skin Lotion / Gels - anything liker this to your penis regularly?  Do you use a Penile Hygiene regime, if so, describe it?

    There are a number of ways you can improve your own situation - but, I need more information as a starting point ...................

      Thanks for responding.

      I meant I have had the tight hard to retract foreskin all my life, or atleast for as far back as I can remember. I'm 28 and the tightness has always been a nuisance to me when having sex, so thats how I know thats not a totally new addition.

      I haven't had a biopsy yet. I'm planning on having one now though. I put it off for a while but now its not responding so well to the Dermovate I want to know for sure what I am dealing with.

      I've been using dermovate twice a day for about 2 months now. I massage it in to each area for about a minute, usually after I've gotten out of the shower.

      diet wise, I'm pretty healthy most of the time. I rarely eat dairy which I know I'm slightly intolerant to. I take B Complex, D3, Omega complex, Vit C and a Probiotic everyday. Also, since I found out I'm borderline deficient in Vit D and B12 I've started a course of injections to boost my levels even quicker. 

      I use condoms when I have sex yeah. I had some rough sex last week and that made the situation a. lot worse and irritated the glans quite a bit.

      My hygiene regime is - I clean the area every morning and night in the shower with a cream my doctor gave me thats extremely bland and made for that area. Then whenever I go to the toilet I make sure there is no urine left anywhere.

      I guess the bits that are baffling me about the LS diagnosis is the lack of white patches that are so iconic of LS. Mine is just very red. Also the pictures where it came up in raised red spots.. I've never seen anyone mention that happening with their LS. Its annoying that it responds so minimally to the dermovate because as far as I'm aware thats the only official treatment available for LS. I'm pretty worried that if it is that then my condition will soon worsen even whilst I'm doing everything I can to keep it at bay.

      I think the biopsy is definitely needed. Any ideas as to what else it could be?

      No probs: what I would suggest you do is treat this as a LS type condition anyway until a Biopsy proves otherwise.

      The raised red spots you mention look to me like a severe dermatological reaction to something your glans / exposed foreskin is in regular contact with.  The most dangerous time for an adverse reaction to happen is when you are asleep for a number of hours. This is when you need to ensure there is nothing whatsoever in terms of any form or chemical left under your foreskin that may have got there during sex, masturbation, or simply following showering / washing your penis.

      I have no other suggestions as to what your condition could actually be.  If you are are concerned about preventing your condition worsening this is what I would do:

      > No oil based products (mineral or plant), no soap, no Lubes, ointments, creams, hair shampoo, body wash etc to get anywhere near to your exposed foreskin or glans / head of your penis

      > use water only to lightly wash by rubbing your exposed glans / retracted foreskin with after you have showered normally - wash this way after sex, masturbation and every time you urinate

      > if you need to retain your condom use find ones for sensitive skin types - it is all the 'gunk' - spermicide and 'easy-on' type chemicals that are positively dangerous to your exposed glans and foreskin, particularly during extended sex sessions when you have this kind of condition

      > wash all clothes, towels etc that come in contact with your penis with natural soap flakes only - you can get 'Liquid Soap Flakes' from most reasonable sized supermarkets - so you can bung all these clothes in the washing machine

      ** the essential is that you address the tight foreskin issues, as, this situation is most likely to worsen with this type of condition.  I combine applying Clobetasol {Dermovate / ClobaDerm} when I massage it in with doing foreskin stretch exercises to retain as much elasticity in my foreskin as possible.

      > I have addressed particular stretching exercises for the foreskin in many of my other posts.  here it what I would reiterate: -

      - always start with gentle exercises using minimal pressure - build on repetitions, pressure and intensity over an extended period of months, rather than days or weeks

      - there are two simple main stretches to do,  during / immediately after a shower: i.) draw the entire skin of the shaft of the penis towards the body, this is best done if you are partially erect rather than flaccid or fully erect.  AGAIN, start with minimal pressure - add the amount of time in seconds you hold this for, over a period of months; ii.) grasp the loose foreskin with two fingers and draw the entire foreskin as far forward as possible.  IF your foreskin is particularly tight you may not be able to achieve much movement forward away from the glans.  gently does it adding repetitions and the amount of time you hold the stretch for over a period of extended months.

      NOTE: I had Phimosis what I was about 17 YOA, it actually took me until I was in my mid-fifties to achieve full-movement in my foreskin and this is when my penis took on the typical helmet look of most exposed male genitalia. = It often takes time to achieve what you may consider normal!

      Hopefully you will get a Biopsy sooner, so that you can have a definite on what it is you are actually dealing with ......
