Penis inflammation for 14 months, at wits end please help

Hi there, had this on going for 14 months or so now, I believe it started off as a friction wound with a fleshlite. Been back and forward to sexual health clinics, GP, walk in centre, Urologists, Dermatologists no one knows what it is. Here's a list of meds I've used:

Daktakort - from the Urologist

Clotrimazole - from SHC

Moisturising wax - from SHC

Fluconazole - from the GP, 2 week course (5 tablets or so low strength only 150mg)

Ciprofloxacin - from the walk in centre from when I was peeing constantly (believe it was a prostate infection) cleared it up took for a month but inflammation still persisted

Hydrocortisone - around a month or 2 in self prescribed (stupid I know) only used for a couple of weeks

Protopic Tacrolimus Ointment 0.1 - from Derm, currently 2 weeks in no change

  • Things worth noting: I was sure it was a candida issue as I have a white tongue in the morning and I've only had it since having this problem, I have to scrape every morning. I also have lower back pain, the only time this has looked better was when I did a yeast / sugar free diet for a month and it did improve a bit but not fully , returned soon as I ate normal.
  • Everything that comes in contact with it disturbs it, such as pee - I am hygienic I wipe every time I go the bathroom I don't use shower gel or oils etc literally nothing touches it apart from pee and prescribed creams.
  • I did a 3 day water fast to reset my immune system last week believing it may be an auto immune disease, no improvement since but obviously it's only been a week

Depressed and anxious over this I want my life back to normal again, I have read on another post from a guy with the same problem and exact circumstances that he used an anti biotic ointment and it went away, so far I have only used anti biotics such as Daktacort which I read isn't completely anti biotic, I believe I have an underlying issue due to the tongue scraping, lower back pain and previous balantis/ yeast infections, why else would I get a prostate infection? I have read you get those from untreated yeast infections, not sure if I need a longer/ stronger course of fluxaconazole mixed with an anti biotic ointment thats my best bet.

Any advice would be greatly appreciated

    Have you seen a Dermatologist who is a specialist in Genital Skin Disorders? You have obviously endured quite a lot for over a year now - it is time a medical professional helped you get to the bottom of what is happening for you with this ......

    If you are getting re-occurring yeast / fungal infections you need to find a way of breaking that cycle. So the best possible personal hygiene is required with everything that goes with that. These are the kinds of issues and practices that you need to look at in fine detail to see if there is anything you can do personally to change for the better what has been happening for you.

    What do you wipe your penis with?

    In all, how long would you say you have been applying prescribed creams? The Daktacort you were using is a mix of hydrocortisone ans antifungal agents - both elements may not have been strong enough, so the infection may not have been fully dealt with, and, the impact of the steroid element may not have been sufficiently strong enough to enable your skin to begin to heal and fully repair itself.

    After that, you need to look at your sexual practices, such as are you regularly sexually active? Do you masturbate regularly, and, the important thing is do you do a full clean up following ejaculation ..... ?

      Hi there thank you for the reply, I only recently seen a derm who specialises in genital skin disorders he's the one who gave me protopic tacrolimus monohydrate which I believe was in case this issue was lichen schlerosis or zoons, it had no effect what so ever. I believe I gave my self fungal infections along the way in my cleaning habits for example I went a week with out pulling the foreskin back to see if that would lessen the inflamation but then by doing so gave my self balantis, This has happened maybe 2 or 3 times in the last year the last of which was 3 months ago or so and learnt not to do this again lol.

      Other than my experimenting gone wrong I am otherwise very hygienic, I make sure there is no water under the foreskin after showering I completely dry it, I wipe with toilet roll every time I pee religiously so theres nothing coming in contact with it. To answer your question each cream I have been prescribed I only used for a course of 2 weeks and not longer. Sexual practices I don't use any lube or anything that would disturb it further, and with out being too graphic I am down to once a week or 2 with little to NO friction at all, I don't want to be too graphic but I am so minimal barely even touch the thing (havn't for months) I don't think this is just a skin issue I believe it's something underlying not just simple inflammation

      It's funny you say daktacort is anti fungal I believed it was anti bacterial as well atleasts thats what the urologist told me, I'm starting to believe this may be a bacterial infection causing the inflamation? as all the creams I have been using the last year or so have only been anti fungals, there was a guy on another thread who said this:*

      It all started after I used my fleshlight the second time. I didn't clean it after the first time and left it overnight (including my penis). Gross, I know. I was punished for a year after that and it's a lesson I'll never forget.

      Identical circumstances so I'm leaning towards bacterial infection, I had a prostate infection 78 months ago which could have been a result of this. Also the only time it has looked better was when I did a month of no sugar (I have read bacteria will feed on sugar) and no response for fluconazole and anti fungals makes me believe it's not a yeast issue.

      There are steroidal applications that have both antifungal and antibacterial agents, the one I know of is trimovate.

      If there is a suggestion that you may have a severe genital skin disorder such as lichen sclerosis you should have a biopsy to prove what is present, or at least you should have tried one of the potent steroid ointments such as clobetasol (dermovate) for around 3 months to see if this help the damaged skin to recover.

      Recovery will ONLY occur once all sources of irritation have been removed, irrespective of what form of treatment you are using at the time ......

    i think you have had this long enough now to maybe warrant a penis biopsy mate. providing you have followed the right treatment for the right amount of time and it is still persisting could mean its a more severe genital skin condition (early stages of bxo) lichen sclerosus. you need to find a genital skin dermatologist.... your local gum clinic will most likely know a contact.

    in terms of creams, trimovate is amazing. it is a 3 in 1 cream - antibiotic/anti fungal/steroid that works wonders. it cleared my balanitis up after a two days of applixations (twice a day).

    but honestly, try and find a specialist. also get a swab done on the glans to check for bacteria.

    thank you for taking the time to reply guys. I do actually have and have used trimovate sorry i forgot to mention it, only for 2 weeks though I do have some left. I haven't applied any of the creams for longer than 2 weeks maybe thats why nothings been fixed if you believe it may need 3 months. Also regarding irritation literally NOTHING goes on it at all i mean soaps gels anything from the shower or anything else, literally just water and the creams I have been prescribed which is probably 1 different cream for a 2 week period every 2 months or so.

    Regarding the skin biopsy yeah I've been putting it off tbh hoping it would get sorted before that as I was quite anxious to get it done but think i'll book it in, scared it'l hurt but obviously its better than what I've got. I also asked sexual health clinics to swap several times all said no? with excuses like oh we don't swab anymore or theres nothing to swab etc, I'll look into that too. Has anyone had experience with getting a skin biopsy done? do they do it raw or do they apply some sort of cream that'l take the pain away? & hows the pain?

      Sometimes Biopsies provide the ONLY means of moving forward and getting the right treatment put in place - they will likely numb the area then it is simply a case of keeping it hygienic until the skin heals .....

    I found this online yesterday:

    ***This is not a serious problem and it is not a sexually transmitted disease. But it often is difficult to settle down, because just when it starts to heal, when you get an erection the foreskin stretches again and gets little splits in it.

    These then heal with a little scarring, making the foreskin less able to stretch as much next time. This results in it getting tighter, and a vicious circle results.

    Made me think with what you said about all types of irritation, the only time this looks worse is after I've masturbated no matter how little friction and how short it is (sorry to be graphic) maybe the erection is stretching the skin so it can't heal. Dya think vaseline in the morning and at night to avoid knocking it may help and lay off beating the meat? despite how un-frequently it is it may still be causing damage I guess, thats the only time this looks bad or worse not off any creams etc as i don't put anything on it

      It is impossible to know what the underlying condition(s) were on this and what other circumstances were also present - this is nothing more than a generalisation that may or may not apply to anyone else depending on their own personal circumstances.

      The only thing that will actually to anyone that has penile skin that is compromised, is irritated, or has a actual condition present is that sex and / or masturbation will make it worse.

    hi mate thanks for your reply, quick update had an anti bacterial cream prescribed from my private derm, been nearly 2 weeks no change at all (flucidin) maybe its not bacterial, was reading on this post https://patient.info/forums/discuss/ongoing-redness-on-foreskin-and-glans-for-7-years-728587 about using man oil from amazon, a product that supposidly fixes the tears n redness etc of your penile skin, anyone tried it? rekon it might be worth giving ago as im out of options tbh

    also went an NHS derm last week who said she couldnt see much of a problem, gave me a barrier cream n said stop putting stuff on it leave it alone and just use that but I can't see it doing much as I've pretty much done that anyway in and out of me using prescribed creams with no effect

    Have you find out what you have? I have a dry skin of the glans with very little improvement, its one year of this problem, Im tired. I've been to 7 doctors, that said it's nothing and said that I have to put on NOTHING. Except for a dermatologist (the first that I saw) that said it was lichen sclerosus (but was crazy, because he said that used the name of the pathology just to indicate the dry skin of the glans!) and gave me an anti-everything cream called DESAMIXIN EFFE (steroid, antifungal and antibiotic cream) that made the situation worse. Now I'm having an improvement after months of nothing on the penis, even soaps, and the last dermatologist that I've seen (expert in lichen sclerosus) said that mine is not lichen, but doesn't know what it could be, he just supposed that it could be a trauma balanitis, worsen by the steroid cream. Now I'm using only Vitamine E in pills and Elage, that is an emollient cream, since one month. I'm feeling better, even without creams, but there is still a little bit of dry skin and a sticky foreskin, not so much, but I'm tired.

    DOES ANYONE FIND A SOLUTION OR HAVE A SITUATION SIMILAR?

    epiderm cream helps mine. all other creams been a problem made it worse. am so depressed with it all . tried so much

    Hi, I'd just like to say that alot of us guys are worried about simular problems, me included.

    I was heavily sui***al at a certain point due to simular symptom.

    Iv done multiple tests and nothing gets flagged up. But I'm still convinced there's something.

    But there's nothing else I can do to determain what.

    It's nice to know ur not alone, and all I can say is try not to worry, if it was something bad it would be flagged up.

    there is a new product coming out that was called vt-1161 in the testing phase. you can do a search for it and you will find it. I think the name they will use for it as a oral tablet is oteseconazole . I think my spelling is pretty close if not exact but as I said you can search for the testing name. The company is gearing it totally to women, but I think they will find that many men will also want this product. It was very hard to create it and they should probably get a Nobel prize for it. from what I have read it should be available in early 2022. Best of luck to you.

    I have been having the same problem for over 6 months now with not much luck. I have taken a tone of antibiotics with no luck. I think the bacteria has gone to my testicals now. They are very red and painful sometimes. Emuaid helped a little. I started taking frankincense oil and mixing that with Emuaid with better Success. frankincense by it self helped me as well. I am applying it on the inside and outside. It make Take some getting used to but I use a Q-tip applying it inch or 2 on inside. I have been doing this for couple weeks now and the pain has really came down. It burns a little when peeing but thats about it. I hope this can start heeling something soon.
