Persistent Exertion/Pressure Headache

I've been getting daily headaches for the last 2 months and nothing I (or the doctor) have tried has helped so far.

The pain is always at the back, right hand side, near the bony part of the skull in line with the ear. It seems to be triggered by exertion, namely sneezing, coughing, straining or bending over. The initial pain is quite intense and I'm then left with a pulsing type headache and eventually a dull ache which can last for hours. This happens on a daily basis.

Paracetamol and Ibuprofen don't really do anything, neither does massage of neck or hot compresses.

The GP has now prescribed Propranolol 40mg twice a day.

I'm not convinced my GP really knows what to do about it now. Should I be asking for a referral ?

    Might be Occipital Nerve related. Muscles will aggravate the nerve causing headache.

    Try a sample of a triptan - Sumatriptan (Imitrex), Relatriptan (Relpax), Naratriptan, Rizatriptan, Frovatriptan...

    Lidocaine nerve block is great diagnostic test and very safe and quick. Very temporary too but if it alleviates the pain you know the cause.

    Injection of small amount of lidocaine or similar into the area where the Occipital nerve transits the muscle at the base of the skull on the nerve's path under the scalp.
