Persistent Toenail damage and now nail deformation

Hi there,
I play sport all year round (Cricket and Football), and about 10 years ago got trodden on and lost the nail. Since then, through general wear and tear, I probably lose the whole nail again every 12 months on average, normally after a minor knock, starting the cricket season (i'm a fast bowler so lots of impact), or if the ground is too firm at football.
Over time, the nail has grown back more and more deformed (Dented and uneven) and has thickened to the point where cutting it is becoming impossible. This has lead to me bowling this weekend, and I'm pretty sure I've separated the Whole nail yet again. (Completely black, red swelling and throbbing yesterday, tender to touch and has that numb feeling when it dies).
So I want to get something proper done about it - but I don't know how to go about it, and realistically what can be done. There is a Podiatrist near me, but there's nothing on their website about serious nail damage, only ingrowing toenail surgery (That was the most gruesome thing I could find). Should I look visit my GP, or is that just going to be a waste of time?
I have no idea who the best person to speak to about this sort of thing is...so any advice would be greatly appreciated!!!
