pH manometry

Just been diagnosed with Barretts Oesophagus, duodenitis and gastric polyp. I have been on Omeprazole for 10 years after suffering years of heartburn (3 pregnancies each time heartburn started earlier but never went away after last pregnancy22 years ago) I had pH Manometry (acid levels off the scale) and endoscopy 10 years ago when hiatus hernia was discovered. I was referred to surgeon who recommended nissen fundiplication warning if I didn't have it I was risking Barretts. I got a second opinion and that surgeon said if medication was controlling symptoms then don't have surgery, so I didn't, I have just been given repeat prescriptions since then.  At the end of last year I read that it was not recommended to be on Omeprazole for an extended period and I was also having problems with my stomach, burping continuously and not feeling great so I asked my GP why I hadn't been followed up, so he referred me back to hospital and I was sent for endoscopy where Barretts was discovered.  I am now having another 24 hour pH manometry on Tuesday and so have had to come off my Omeprazole a week before, currently suffering severe heartburn. Just wondering if nissen fundiplication would be any good now the damage has already been done? Omeprazole clearly helped with my symptoms but masked what was actually happening to my Oesophagus

Not sure how to repond.  I've not heard of nissen fundipication.  Are you coming off the Omeprazole for good?  I talked to my doctor about not taking it if reflux stopped.  But he said that since I have the barret's I need to keep taking it.  I discussed risks, but he said the risks of not taking it are worse than the risks with taking it.  Have you taken any natural remedies?  I eat manuaka honey (from New Zealand, needs a rating of 10 or better), DGL licorice, green tea and chamamille tea

no I am not coming off the omeprazole for good, just have to stop taking it for a week before the pH manometry test so that my acid levels can be measured. just wondering if it is worth having the nissen fundiplication surgery now.

Sunlover  sorry to hear  that  , I can give you some informations about Barrett surgery all i read  on internet  . you can check on Google  Nissen Fundoplication for Barrett .  they say Nissen Fundoplication is not indicated for Barrett but to control GERD symptoms . when high-grade dysplasia is discovered and confirmed by a second pathologist , endoscopic ablation is the standard of care . hope you dont have dysplasia . Barrett ablation is to eradicate bad cells . take care of you

    Sunlover,

I had diagnosed GERD, then Barrett's w/ wide range dysplasia, then hiatal hernia, then esophageal motility disorder (Nutcraker Esophagus), then doudenitis, then gastroparesis - in that order (over time).

The problem is one must control  acid reflux (thru use of Omeprozol etc.) yet prolonged use makes it so your food cant be properly digested due to lack of acid production.

Your in a very dfficult place indeed!

Thanks for replies, we seem to be in a catch 22 situation, can't do without the medication but the medication is also causing us problems.  I have also read about endoscopic ablation so will be asking about that and also whether nissen fundiplication would be indicated now.  24 hour pH Manometry  is happening tomorrow! can't wait for it to be over so can get back on the Omeprazole, Oesophagus currently on fire!

pH Manometry underway. Have to say procedure was more unpleasant than I remember from last time 10 years ago. Had tube inserted thru nose to measure pressure when swallowing water, after this was taken out another tube was inserted which I had a bit of difficulty with and had to remove it and try again. A bit traumatic I think because have had a bad week with the heartburn and lack of sleep. Tube taped to my face and attached to device called a digitrapper. Now having to write down times when I eat and record when I am eating and when I am experiencing heartburn on the digitrapper. Need to eat foods that trigger heartburn to get best result. Apparently normal levels should be around 14 and last time mine measured 52 and then 84 for the second test so will be interesting to see what they are this time.....

Just been to have tube removed following a very bad night. Didn't sleep much especially as instructed to lay flat and after having to eat foods that would aggravate and provoke heartburn such as curry!

Heartburn was very bad, was desperate to get the tube out this morning, now back on Omeprazole but guess will take a while before I feel comfortable again. Now have to wait for results and appointment with consultant, however was annoyed to find there was letter in my file saying I had already had nissen fundiplication surgery 10 years ago when I haven't, it was only recommended, will have to phone up secretary to ask for that to be corrected as don't want that mistake to influence what happens next.
