Phagophobia analysis

My cousin 30yrs old with no history of depression complained pain in the shoulder and admitted to a hospital nearby and while going to hospital, i gave him some water to drink and he couldn't drink it as he felt like choking. The doctors gave some pain killers and the shoulder is fine. After reaching home, he didn't want to eat or drink anything as he felt like he cant breathe if he drinks anything. The same night he complained that he can hear his heart pump and it doesn't feel right. We took him back to hospital and Dr said its some kind of anxiety and he needs a psychiatrist as his ECG was also normal. The next day it got worse as he couldn't eat or drink anything. We took him to another hospital where an ENT specialist checked and confirmed that there is no issue with his throat and its just anxiety. The next morning my cousin started seeing things that others couldn't like the lights falling down, objects floating around and he acted like he has some mental health issues. He kept shouting, crying, screaming and being more emotional than normal. He did not support for a brain MRI as he complained that the noise is unbearable and Dr couldn't give him sedative to perform MRI as he was already in a sedative. CT scan reports showed nothing abnormal
The next morning he kept screaming and his heart stopped functioning and died unfortunately. What I am looking for is if there is a possible explanation on what this could be as it happened all of a sudden in 2 days and is this a phagophobia which became worse or did we/doctors miss something here
