so sorry to hear you're still experiencing issues back there. i am 9+ year chronic fissure here myself , along with on and off internal extrnal hemmorhoids, constant constipation, pelvic floor dysfunction. i have used all creams/compunds under the sun and they used to work alonside pelvic floor therapy but more recently had EUA fissurectomy and botox. on month two still healing being positive and taking it day by day.

not sure the extent of the incontenince but usually after each BM or occurence i do use a bidet and then A warm to hot sitz bath (the one that goes in/on the toliet) for 15-20 minutes. this usually helps relax the muscles back there and increase the blood flow to help minimize the hemmoroids. when i get itchy back there the culprit is usually moisture of some kind - i make sure to fully dry everything back there with either hair dryer on cool setting andor bunched soft tp placed and left till dry or pat dry very gently - no wiping or any kind should be done back there. i also use B sure pads when hemorrhoids are causing issues. these catch andy if the mositure back there. diet is very important and plays a big role in my issues.

i hope things get better. take care
