PI-RADS 5 MRI prostate score and follow-up negative in-bore biopsy

At wits end...with MIXED MESSAGES.
**I'm 68 w/ a 15-year history of elevated PSA ...most recent a 6.0. Had laser treatment 14 years ago and later a TURP 7 years ago for what was always diagnosed as BPH.
I've had FIVE biopsies since 2014...ALL negative.
**My 1st ever MRI in October 2018 was a terrible PI-RADS 5 suggesting "clinically significant" cancer.
Now terrified that I've had PC all along, I had a fusion biopsy in December (16 cores)..again: NEGATIVE! (I even had a RE-check interpretation w/ Johns Hopkins providing the same NEGATIVE biopsy result!)
**Searching for an answer, I researched IN BORE biopsies which (supposedly) are more accurate (90 % ?) than "fusion" biopsies that use a previous MRI NOT done IN bore. Unfortunately, the IN BORE procedure is not available in AZ where I live but IS available in FL & CA. I chose to drive 400 miles to Indian Wells CA 3 weeks ago and paid "out of pocket" for the IN BORE biopsy... now convinced I have PC.
**RESULT: 5 "extremely targeted" lesion cores were biopsied: ALL NEGATIVE!**Are the "lesions" really malignant? Were they MISSED despite the extreme targeting? Was the PI-RAD 5 a "FALSE" reading?
My ongoing dilemma: Is it PROBABLE that all my negative biopsies - ESPECIALLY the recent (supposedly) innovative IN BORE biopsy...are accurate and the PI-RADS 5 MRI is not accurate?
**My next "regular" urology appointment is in October. I give up...

unfortunately, diagnostic MRI and targeted biopsies can (rarely) be false negative, which can be frustrating as in your case .
You might have another expert review your MRI's as a third opinion
