Pimple on internal hemorrhoid (pics attached)

I have a small pimple like bump which has appeared on my previously diagnosed internal hemorrhoid. I fear that it may have been nicked by a fingernail a few weeks ago during sex.

Our doctors are closed except for emergencies, so my GP called in Keflex (Cehpalexin) antibiotic and I have completed a 7 day course but the pimple is still there.

Does anybody have any idea what this is or what I should be asking my doctor for or telling them to treat it? The last thing I want is for this to become a serious infection.

    That is a Anal papillae , common with hemorrhoids
