There certainly are such things within the realm of Non-Obsteuctive Coronary Artery Disease (CAD); Microvascular Dysfuncti0n and Coronary Spasm for example. Both are difficult to diagnose with typical cardiac testing and gets missed. Especially in men. There had been older Research claiming this was mostly a women’s disease but in reality - 60% female and 40% male. Non-obstructive CAD is rapidly increasing as a problem. Interventionalist cardio are used to fixing and even curing heart issues.... these tricker types are not as easy to understand. But there is hope of you simply get the correct diagnosis. Often at a hospital of excellence (teaching ones preferable seem to be more in the know).

    Google search:

Coronary Microvascular Disease- Non-Obstructive CAD or Coronary Spasms

and see if any of this fits you.

Symptoms are often at rest, at night or during very simple activities. Often with a delayed response to exertion.

Best to you- hoping you find out what’s up. Anxiety sure can disguise itself as cardiac symptoms... it is optimal to always address anxiety and learn ways to cope or meditate. However if nitro ends up alleviating the symptoms and it is at any time regardless of anxiety? They may look to other roots for this. Hard as dismissed symptoms ALSO lead to anxiety...

i went on undiagnosed for many, many months and it took some searching to get to a good, curious and knowledgeable cardio and then find local care to partner with him. Now life is much more manageable.

Husband who has heart failure, That would definately be picked up by x-ray and or echo.

Had left arm pain, weakness, for 15 months even knowing about his pre-existing heart failure junior cardiologists at heart hospital suggested it was muscular, arthirits of shoulder and or even rotator cuff injury, all of these were checked by ultrasound, x-ray and even cat scan and all came back clear, left arm pain and weakness continued.

It turned out to be an intermitant or failing right branch bundle block, he already had a left branch bundle block.

One morning woke up at home told me he felt really weird, he didn;t look good, so I used our Omron, Blood pressure normal, but heart beat at 30BPM, took him to heart hospital.

Even there with heart at 30BPM, young Dr suggested to me pinched nerve in his neck causing left arm pain, and removed some of his heart meds as apparently those meds slow down your heart, thats exactly what they are supposed to do, heart beat then dropped to 23BPM in the middle of the night, he was on monitoring in the hospital, so picked up pretty quickly.

Saw senior consultant the following morning when I gave him a run down of the previous 15 months, the pinched nerve in neck the previous day, withdrawl of meds, told him his junior staff were idiots and needed an education.

Consultant turned and growled with the scowl from hell, at his junior staff and ordered them to his office NOW, nurses with eyes like saucers.

Consultant came back about 20 minuets later and told us a pacemaker would be installed later the same afternoon, it was, and now nearly two years later husband doing well.

Husband has 100% branch bundle blockage, so the three lead pacemaker replaces the lack of natural electrical pulses to make his heart beat.

You may need a halter monitor for a week, to record what your heart is doing, a few minutes on an ECG machine are not enough to catch failing electrical activity.

Other than than a fitbit, where you can download the results, or ECG, during one of the episodes, does it look different to normal times when you have no pain in arm, if so you need to show Dr and or ER room the results of fitbit recording, if we had that we would have caught the failing electrical activity before his heart dropped to 30BPM.
