Findings:

Menisci: An elongated multiloculated lobular T2 hyperintense cystic structure is seen anteriorly in the tiboifibular joint and is seen adjacent to the ACL insertion and anterior horn of the lateral meniscus. The cystic structure measures approximately 3.3 x 0.7 x 1.2 cm in greatest transverse, AP, and craniocaudal dimensions, respectively. No discrete underlying lateral meniscus tear is identified. Therefore, the structure is thought to more likely represent a pericruciate ganglion rather than a parameniscal cyst. The medial meniscus is intact.

Ligament/tendons: The extensor mechanism, medial collateral ligament, anterior cruciate ligament, posterior cruciate ligament, and lateral collateral ligamentous complex are intact.

Articular: The articular cartilage is normal in signal and morphology. There is no joint effusion.

skeletal: there is no evidence of acute fracture, marrow, replacement, or osteonecrosis.

soft tissue: the remainder of the soft tissue structure surrounding the knee are normal in signal and morphology.

Impression

An elongated cystic structure is seen anteriorly in the tibiofemoral joint which is thought to most likely represent an ACL pericruciate ganglion. No discrete anterior horn lateral meniscus tear is identified. however, the structure representing a large parameniscal cyst arising from a nondiscrete lateral meniscal tear is a somewhat less likely differentail consideration.
