Please, what could be causing my perianal dermatitis?

For a year and a half I have itching in the perianal area, three months ago I went to the dermatologist and they diagnosed me with perianal dermatitis, but he only prescribed corticosteroids that gave me only partial relief and we did not find the specific cause of the dermatitis; Then I went to the proctologist and he ruled out any internal problem but he agreed that there was irritation and he told me that it could be anal herpes; Finally I went to a second dermatologist who told me that it is not anal herpes and that I still have irritation in the area and he prescribed zinc oxide which I have been using for four days. Please, Do you see any anomalies currently?
