Possible miscarriage?

my girlfriend is believed to be 5 weeks pregnant, 6 days after when her period would of been due, she started suffering some pinkish bleeding, by the night time the blood was bright red, but wasn't as heavy as a usual period, she was only need to change her pad every 6hrs (usually only lasts 2 hrs during her period) she does usually have a heavier period, started seeing very small blood clots when wiping on the 2nd day, then on the 2nd night she passed a small piece of tissue which was about the length of a 20p coin, rectangular shape, and made a small clink almost like a coin when it hit the toilet, is there any chance this isnt a miscarriage or should we fear the worst at this point? thanks in advance
