Possible Prostatitis? Need Help!

Hi

I hope you guys can help me as I am struggling at present.

I am a 32-year-old male, 5 foot 10”, normal healthy body weight.

I am looking for some advice as to what I could be experiencing, if it is prostatitis and where I go from here.

I am going to post everything just so it helps with understanding what may be going on!

At the age of 26 I began taking Propecia (Finasteride) for male pattern baldness.  I took the tablets once per day for (1mg) as prescribed by a well-known high street company who specialise in male pattern baldness.

I took these tablets for around three to four years.  One of the first things I started noticing was suddenly I began to get clumps in my semen, they looked like little jelly-like clumps in amongst the liquid… no unusual colour, but jelly like in nature and they look almost like little grains of rice mixed in with the normal liquid semen.  It was very odd and freaked me out a little.  I mentioned to my GP, who informed me that semen comes in all types of consistencies and I had nothing to worry about.  I wondered if it was to do with the Finasteride but he reassured me he didn’t think so and I had nothing to worry about.  I must also mention that my hair responded extremely well to the treatment and looked great, nothing wrong with it at all, I was so happy!

However, after 3 to 4 years, I did notice that I had a lowered libido and my girlfriend also worried about me taking it and if it could cause any long-lasting problems for me.  We agreed I would stop taking it.

After a year to eighteen months or so of being off the Finasteride, my hair began to thin again and this caused me to get depressed and worry (I hate it!).  My girlfriend said if it made me feel better, try the Finasteride again as she could see how much it was making me sad.  So I started taking the tablets again, but just once every other day (i.e. 1 mg every other day).  Again, my hair got much thicker which made me far happier.

However, after about two or three months, I started getting a pain in my left testicle.  It didn’t hurt if I touched it, but was just a dull aching pain that was there all of the time. I went to my GP, explained everything, he did not believe it was due to the Finasteride and told me he would refer me to a urologist.  In the meantime, I was given two weeks’ worth of anti-biotics (Ciprofloxacin) and sent on my way.  This never really got rid of the testicle ache.

My appointment eventually came to see the urologist.  He gave me a digital exam to feel my prostate and said it felt quite normal but he thought I may have prostatitis.  He told me to see my GP again to get a six-week course of Oxytetracycline.  He also booked me in for an MRI scan on my testicles.  I also asked the urologist about the Finasteride and he said that because I had taken it for a few years, he did not believe it was that.

I had my MRI scan two days later and it showed I had an infection in my left testicle (orchitis).  As a result, I was referred back to my GP for anti-biotics.  Upon seeing him, he said he didn’t think he should prescribe me Oxytetracycline as that’s for a prostate infection or inflammation and that wouldn’t clear up the infection in the left testicle.  I enquired as to whether the infection could be a result of the Finasteride or as a result of prostatitis and he said he didn’t believe so and I could keep taking them and he saw no reason for me to stop.   As a result, he prescribed me a three-week course of Doxycycline.  I took these and after the three weeks, the testicle ache remained and was no better.  I returned to my GP and he this time prescribed me another three weeks’ worth of anti-biotics, this time Cefalexin.

Finally, after three weeks of these anti-biotics, the testicle pain went away.  However, about a week into taking these anti-biotics, I began getting some new symptoms that have not gone away and this has now lasted for around two months.  I am getting a tingling / stinging / cold / wet type feeling in the tip of my penis.  It is pretty much constant (can feel it now) and has been there every single day, bar about two, for a couple of months now.  I notice that when I urinate, it doesn’t sting, but afterwards it seems to get worse, like urinating has irritated the tip of my penis.  It is a weird feeling, sometimes as weird as it sounds, it even feels a little like something is ‘stuck’ in the end of my penis in the tubes!  The penis looks normal and isn’t sore to touch or red or inflamed, but this tingling sensation just will not go away.  I spoke to my GP about it on my last visit, informing him that the testicle ache had gone but that I was now getting a strange tingling / stinging feeling in my penis tip and he said it was odd and was probably a result of the anti-biotics and that the Oxytetracycline shouldn’t do it – but the feeling has persisted and clearly shows no signs of going away!  I asked if it could be as a result of prostatitis, to which he replied he did not think so.

It also makes me feel a little like I need to urinate and even when I have been, it still feels a bit like I need to go and just tingles all the time.  I don’t have any trouble urinating, it comes out in a steady stream and doesn’t sting or hurt.  But burns a little afterwards and then continues to tingle.  Sometimes it feels like it is cold as well and a wet type feeling sometimes too.

I am having no problems getting an erection or anything, but it is affecting my sex life because when your penis tingles and you feel at times like you need a wee, even though you don’t, you do not feel particularly in the mood for it!  A couple of times, I have also had very bad stinging in my penis after intercourse and it has lasted a good couple of hours afterwards and makes me feel like I need to urinate constantly afterwards.  Every time I urinate and think I have finished, I have to go back to the bathroom and urinate a little dribble again, and this lasts for half an hour or so.  That’s happened on two occasions after intercourse.

Usually, when I wake up in the morning, I get the tingling feeling, but it seems to get worse as the day goes on.  I sit on a chair in an office all day for my job from 9-5 (like most of us!) and by the time it gets to the evening, my penis is usually tingling and/or stinging like crazy!

I am seeing my urologist again on June 9th, but, in the meantime, any advice that anyone can give would be greatly appreciated.  I go on the holding the following day, so really wanted to get this resolved because I don’t want this to ruin my holiday!  I am not in pain, but the tingling penis feeling is driving me insane!

Thanks so much for any advice.

Lose your hair, keep your girlfriend and your manhood. Is baldness not a sign of virility? Finasteride may even have you growing breasts. Did they check you for Epididymitis as it is quite common in your age group.

      Hi mate

No check for epididymitis... would that make your penis tingle?

Also, if it was that, wouldn't all these anti-biotics have gotten rid of it anyway?

I am just guessing based on my experience. My GP gave me Cipro for prosatitis. I went back after a few weeks with most symptoms gone but still testicle pain. He then said after examination that I had Epididimytis that needed a different antibiotic and is evidently often difficult to diagnose..  No penis pain at that time.

Will sounds like you have a form of prostatitis. Been suffering for 25 years. No known cure. I manage the pain with verious supplements and Advil but as always, given cerntain activities or food I may eat it comes back with with a painful attack.

I am looking into PAE to reduce my prostate size in the hopes of controling the swelling that happens with prostatitis. Bee Pollen might help. I suggest you do researc. Look at the Prostatitis Foundation and start from there in your research.

    good luck.

One of the side effects of finasteride [apart from growing breasts [might be fun] and regrowing head hair is that it can cause retrograde ejaculation which sends [some of] the semen into the bladder which may account for the jelly.

You are a youngster and I'd guess it's partial if that is the case.

Oddly perhaps [I take it with Tamsulosin] and am [ahem!] old I do not find any decrease in libido rather the opposite having had my first nocturnal orgasm [i e while asleep ] for probably 55+ years together with much quicker orgasms and unstimulated trouser stirrings at other times.

Urologist suggests "rebore" i e TURP but I think I'll wait till Urolift more widely available

You may not grow two breasts! Mine was more pronounced on the left side. Ever since I stopped it two years ago I have had an itching tender sensation mainly on the left. My doctor gave me a cream to use. After ten days the sensation dies down but returns when I stop using it.

Hi - thanks. This is a UK website and I am in South West.  the good news is that we get our treatment "free" but it;s sometimes difficult to access things.

Not going to get any easier I'd guess with our new "government"

Simon.  I guess that is a good thing.  You just have to keep on fighting for what you can get.  Canada gets there free to.  Mine was covered by my insurance.  Uro-lift was the best.  after 5 days no blood and sleeping all night.  It is 2 weeks monday and I feel great.  I would never have anything that they would cut or burn my prostate.  Less avasive first  Take care  Ken

Did you ask your urologist what happens after Urolift when your prostate grows more as it will ?

No one who has had it seems to had an answer to that question.

My thought has always been to get rid of the excess gland by the least invasive laser approach.

I am going on monday that is my 2 week visit  I will ask.   I don't what to take that 35% chance of a ED problem.  They case enough problems last year when they tryed to force a catheter in me 3 times never got it in the bladder.  Hurt like hell.  I have been the same.  I trust my doctor and he know me very well

Do you have reliable stats on ED after each surgigal procedure? PVP is supposed to be around 10%. I was OK after PVP but by the time I had Holmium laser seven years later Tamsulosin was already causing retro.

Derek  when I was having the problems and doing the meds I was not happy a dry orgasm was not my thing.  The orgasm was ok I guess but I like it when it come out.  When it went in the bladder it not feel right got a hour or so.  Ehrn my doctor which I trust told me about the turp and the turnis.  I told him no one is cutting anything away from me.  I read up on both and there were a few test done on 100 men with the some problem About 65% were good with both but 35% had some ED problem.  From dry orgasm to not being able to get hard at all. He did not force me to do anything. Thats why my doctor told me about the Uro-lift.  No cutting It 2 weeks and I feel great no getting up at night to pee.  Orgasms are good so I think I made the right pick  Ken

As someone said on this forum prostates are like a weed as they keep growing. Mine was 35 g in 1995 and I refused a TURP to wait for laser surgeries being developed. By 2005 when PVP had been in the UK for about a year I fought the NHS to be referred to a hopital that was doing it. By then my prostate was 75g. To me PVP was basically day surgery.

Done on a Friday afternoon and home by train on the Sunday and out and about as normal on the Monday.

Sadly it grew again and by 2013 when I had Holmium laser surgery it was 130g. That should have been just as straight forward but they sent me home with a catheter in to get me out the next day when I was still passing blood and I caught an infection from the catheter.

I had been pressing for laser surgery for some time and two urologists at the local hospital said that they did not do it...It was no good and that was why my postate had regrown.

One day I had been seen by a locum who told me that a kidney surgeon at the hospital had been doing prostate laser surgey for about six years and referred me to him.

Two years later my prostate is 50g. but Im not sure how much tissue he left after the procedure. The sums are prostate was 130g. He ended up with 36g of tissue to send for analysis with an unknown amount vaporised by the laser.

The 50g prostate is not showing any obstruction on a recent cystoscopy but my flow is very variable from day to day for no accountable reason.

I'm not sure why some prostates grow more than others or what the average is but most seem to need a second procedure eventually or else they just put up with things as they age

Had my followup today.  Talked to my doctor about that.  The prostate will grow for the rest of your life  He told me that the inplants are very strong.  I had 4 done 2 weeks ago  there is a 7 year study that all look good with the growing.  the inplants will still keep the prostate away from the urethra.  He told me that there looking good for at least 10 to 15 years.  Much better then the other surgery's  Ken

Good morning  I have told a few on this site about and sent Chuck P infornation I have his e-mail.  I also found out on the Uro-Lift site that If you read some of the information I got some wrong I posted before that there is a 65% chance of ED.  Thats wrong to a point.  on the Turp there is a 3-8% chance of bleeding and you will need a transfusion 2% with Laser.  7% chance of Urethral Stricture with Turp and a 3% with Laser Incontinence 3% with Both.  ED is 10% with Turp and 7% with laser. Retrograde Ejaculation with the turp is 65% and Laser is 42%.  With the turp and laser it is permanent and cannot get fix.  Urolift is 0% on everything.  Today was my 2 week visit.  I JO 2 time today looking at porn and there was a good amount and it felt great.  Should get better  Take care  Ken
