Post Cataract ghosting/Double vision

I had cataract surgery 8 weeks ago on rt.eye. only. I was still reading 20/25 with contact lens but had bad glare. I had a toric IOL implantednd am now slightly undercorrected in that eye both power and some astigmatism. I struggle to read 20/25 with much blur so the surgeon sent me to optomotrist for refraction. I am now wearing a -.50 contact lens in my surgery eye which gets me 20/20 vision. My left eye has a cataract but with a -4.50 toric lens I am still reading 20/20. So, no plans to have surgery on that eye right now. I can no longer wear glasses at all due to imbalance between eyes. So, I am in my contacts many hrs. each day. my vision with both eyes open seems a bit off...like my eyes are not working together. In low light I sometimes see ghosting or double vision only on high contrast items like lit store signs, LED light on alarm clock, crawl on television. This is with both eyes open. sometimes if I use artificial tears I can get the images to meld. The closer I get to the high contast writing the better. My near vision with readers is clear and not ghosted. Other objects in the room are not ghosted...just the lit writing. I feel like the image in my right eye is slightly larger than the image in my left eye and this might be causing the ghosting? I am thinking I will have this issue until my left cataract is removed or unless neuroadaptation by my brain occurs. Has this happened to others after having only one IOL implanted? Is there any solution?

Also, i have worn contacts for 49 years. My surgeon did not have me remove my contacts for weeks or even days prior to mapping that is done before surgey. Wondering why and if this is why my IOL is not accurate.

    there is contact lens induced astigmatism which is why they ask you to stop wearing the lens for 2 weeks before measurements.

    under correction is generally better than over correction and will give you better near vision.

      sadly, they did not ask me to leave contacts out at all before measurements. I am at a large well respected clinic in the Chicago burbs and second guessing my choice of surgeons.

      any thoughts on the ghosting issue?

    I had cataract and multifocals back in December. My eyes keep getting better. Its been 4 months now.

    So, you think my vision may improve over the next couple months? It's 2 months for me.

    Are there others here who had only one eye done and have ghosting or imbalance issues?

      i have only one iol and the imbalance that u talk about is called anisometropia. my other eye is -2.5 so the imbalance is not too bad.

      i have ghosting on tv subtitles but my main problem is positive dysphotopsia.

      yes tv subtitles and for me some lit things like alarm clock numbers. what will happen when we get the other eye done? So sorry you have positive dysphotopsia. what IOL do you have? Was it a toric?

      No mine was not toric. But it was a Symfony. Once the other eye is done the imbalance will go away.

      My fear is that if I get dysphotopsia with the other eye too, then will I see a cross or two arcs crossing each other. Right now there are just arcs from one eye.

      i understand your fear. i am fearful of doing the other eye too and putting it off for as long as possible.

      Wondering if surgeon confirms it's due to eye dilating beyond IOL. If so may be a time thing and resolve itself. Would be nice if it resolved itself before 2nd surgery.

      I am putting off too but the cataract has entered my line of vision. Still correctable to 20/40.

      DANG, that is not good. i'm still correctable to 20/20 in my non surgical eye. i was only at 20/25 in my surgery eye and regret doing it. i saw better before. will not do my left eye until it is really, really bad. 20/40 is getting there:(

      Yes - not that there are many benefits to getting older - lol. But that may be one!

      Hudsongrl - really surprised any optometrist or surgeon recommended cararact surgery if you could see 20/25 with glasses. Here in Canada that wouldn't be covered by our medicare system. Eyesight has to be 20/40 or worse (benchmark for passing vision test for driving lic).

      I think my lens s not centered after constriction. I say that because I can see the concentric rings if I want to. The way to see them is: put a drop of water on the glasses and then look at a light. I see the lens and the concentric rings on it. now the concentric rings should be concentric to the constricted pupil and I don't think mine are. especially in the area where i see the arcs.

      It is also possible that the pupil is not exactly circular but there must be some way to push the lens upward in that direction and my issue should get resolved.

      just sent you a pm with link to article. An update on Symfony. What you mention about lens centering is mention by one of the surgeons.

      The 4 doctors I have seen post-op say the lens is centered perfectly. I think they are wrong though. There is some centration along visual axis that needs to be measured.

      Really hoping you can find an ethical doc. Hard to know who to trust. Do all 4 work in same office/company (any affiliation). They likely cover for each other if so.

      Not sure where julielyn's doctor is but he might be someone to see. Found her lens wasn't properly implanted (haptic outside the bag).

      2 of them are each other's friends but the other 2 were independent.

      What were julielyn's symptoms or how do I search her thread?

      her thread is entitled IOL exchange. If you go to search on cataract forums her last reply on her thread was 1 day ago.

      it's a cataract surgeon that recommended it. he said the cataract was really dense and best to get it out. i did have lots of glare. but, my gut told me to wait.

      I am not an expert but seems to me if you could still see 20/25 the cataract couldn't have been dense.

      Do know not a surgeons are ethical. Did you have to pat out of picket or was it covered?

      wait before doing 2nd eye and if it were me I would seek out another surgeon (several for opinions) before settling on one.

      oh i am waiting on my other eye. i have a history of uveitis and my regular uveitis specialist sent her own mom to this cataract guy. That's why I chose him. I love her...she saved my sight 4 years ago. but, i hear what you are saying.

      Each have their specialties. My own surgeon said before my cataract surgeries that if I wanted an exchange he would not do them. Took that to mean it is not his area of expertise. Given your history he likely should have referred you to another expert to do further assessments before cataract surgery - especially in your case where eyesight was still at 20/25 and not necessary st that time.

      the Clinic i go to has 40 docs. each has a specialty. his is cataracts. he has 18 years experience. i'm not sure he did anything wrong besides slightly undercorrect me by -.50. i have been extremely nearsighted my entire life and high myopes are harder to correct perfectly. yesterday I

      had no ghosting. wondering if maybe my brain is adjusting to the new vision? i see my uveitis doc friday and will discuss all this with her.

      OK the over or under correction thing is within norm. The IOL as it heals shifts back and forth till it settles do eyesight can be slightly near or far sighted from target. Dr tend to target slightly near sighted by .25 diopter as it is worse to be far sighted than near. IOL is much thinner than natural lens which causes the IOL to shift back and forth till it settles and adheres.

      yes the -.50 is within norm. i am hoping as more time passes it will get even better. plus i am still on steroid drops every other day due to my history of uveitis. those blur vision a bit. my main concern was the ghosting. my theory is i am rt. eye dominate, but when cataract caused so much glare, i had to use my left eye for distance. now, with contact in my right eye and cataract gone right is better than my left. like 20/15. so, my right eye should be dominate again , but a bit lazy. i think this might be why i get ghosting in certain light? Rivalry between the two eyes. if that is the case, it should improve as brain adjusts.
