Hi Jennifer,
I did not have any pain after hip op therefore did not need any painkillers. The nurses did not seem understand this and kept forcing tablets on everyone. I took paracetamol for a few days, then thought this is dumb as there really was no pain. Answer to your question no everyone does not take or need narcotics.

I used to hide the Tramadol they insisted in giving me in my sponge bag. I was a bit worried I might knock it over and all these tablets would roll out!

Yes - and no.
For my first THR I was in so much pain that I was taking Oxycodine for a month after the op. Followed by Co-codamol every 4 hours for the next few weeks.
For my second THR I took Co-codamol at night only for a couple of days and that was it. Nothing more was required.
I can only put the different requirement down to the fact that my first hip was severely affected by osteoarthritis - and the operation took 3.5 hours and the second hip wasn't as badly affected and took only 90 minutes.

My advice, skip the opiates. On my 1st hip THR, on hydromorphine, I was so sick, bunged up and lost my appetite, that I forgot about any hip pain.
I just had my 2nd THR and opted out of any kind of opiates including my spinal. Not sure what it is but they use non-narcotic anesthesia and I only used Tylenol Extra Strength in recovery. I did just fine. Pain was under control and I weaned myself off the Tylenol within 10 days.
Don't know where you live but here in Canada we can access medical marijauna. A small dose of THC oil before bedtime helped me sleep immensely and pain free. After awhile, you can easily stop that too as it was not necessary anymore. But it sure worked better than any prescribed pharmaceutical.
Some people - myself included - are quite sensitive to opiods and narcotics and should seek alternatives.
