I'm now 68. I had an MRI in October w/ a PIRAD 5 score. :Indication: "EXTREMELY HIGH probability of clinically significant cancer present."
Had a MRI fusion biopsy in Nov (16 core samples) and all was BENIGN.
Had another biopsy in May...IN BORE state of the art (supposedly) w/ 5 HIGHLY TARGETED cores sampled ...also BENIGN!
FIVE biopsies in last 15 yrs...PSA fluctuates up to 6.0....ALL BENIGN!
The "experts" are NOT!

The key is the experience of the radiologist reporting on the images.
An experienced one concluding PIRADS 4 would be proven right in a biopsy.
A woefully inexperienced one may confuse inflammation with cancer, where the former
is a benign condition.
I write as a 50yo diagnosed with PIRADS 4
