prostate mri pi rad 4

My latest mri came back pi rad 4. previous 2 years ago in september it was 1. should i have a 2nd mri to confirm results or not fight it and succumb to a targeted biopsy. with finesteride my psa has gone down 28 to 12.56. on finesteride about 1 and 1/2 years. All input welcome

    i had PSA 12.0 in June, sent for MRI which came back Pirads 4, so template prostate biopsy end of August, which resulted in a no cancer found diagnosis. Another PSA this week, it is back to 3.1. dont give up hope, it may not be the big C!

      I have reason to believe the mri was a false positive. finesteride has reduce psa from 30 to 12.56. the mri showed prostate size at 180. prosatitus or inflammation would account for both the pi rad 4 and the 180 prostate size. sonagram 2 months ago measured prostate at 106, 80 percent higher. although cancer is possible, declining psa indicate a shrinking prostate. mri 2 years ago little or no chance. working on a 2nd mri, biopsy scheduled for jan. 12. At the very least it's early and need more results. will keep everyone informed, but I wouldnt be surprised if negative.

      It is best to have a MRI without you being on meds.

      Also it's crucial that a radiologist skilled in prostate MRI readings report on it as "generalist" radiologists are not skilled enough for this important task.

      had a second mri antibiodics and ibuprofen 3 days . no abnormalities or malignacies

      Finesteride will cause PSA to drop by approximately half.

      psa was 30 3 years ago prior to rezume. 19.8 after rezume, up to 29 .8 a year and a half later. started finesteride april 2020. down to 12.8 in december. sonagram shows prostate at 106, 3 mri's. 2 years ago 150 no cancer, october 180 pirad 4, december 210 no cancer. asking the radiologist (pirad 4) to review 3rd mri and apply history etc to the new reading. last 2 mri same company different readers. with a falling psa the 2nd reading seemed to be a bit extremes. I know cancer can form even while psa is falling. last psa down 2.5. checked every 3 months. my best guess on volume is the prostate was xl prior to rezum. after it felt like the volume was substantially reduced. since rezum take out cells from the inner prostate the basic structure of the prostate didnt change, like a small lemon with no insides. usually sonagrams are not that far apart. double the size is confusing. no biopsy for now, will try to follow up with new mri around april or may. my urologist said my psa would go to 15 as a marker. guess i'm overachieving. will keep everyone posted

      If possible, try to find a radiologist that is highly experienced in reading prostate MRIs and keep seeing him/her regularly.

      The same images can be read by different radiologists very differently.
