Prostatitis and bladder neck incision - the after effects

Hi all! I'm a 49 year old male and I've prostatitis on and off for the past two years. As part of the investigations he carried out, my uro told me I had a high narrow bladder neck and a thicker bladder - due to the fact it was having to work harder to empty itself. He put me on Flomax for a while but, after a year, I decided to go for the op as I was urinating frequently and had urgency too. My uro thought the op would improve flow and ease my prostatitis symptoms, as he believed the latter was being caused by urine getting forced under pressure into the prostate. Seemed to make sense!

The op went smoothly - in and out within 24 hours - and bleeding stopped pretty quickly but came back 2 weeks later, as I was told it would, due to the wound scab falling off. This time the bleed, although very minor, lasted about 5 days along with greater pain on urination - more pain than I had the first two weeks after the op. Took Cipro, as I had done after the op, for another 5 days in case of infection.

I'm now 7 weeks post op and find I'm still having issues with frequency, mild pain during urination and to add to the fun, I've had an ache in my left testicle for the past 3 months! Saw my uro a week ago for a follow up and he seemed disappointed things hadn't cleared up as well as he'd hoped. I'm now on a course of Doxycycline for the testicle pain (he thinks it's epidiymitis) but 7 days into a 2 week course, they've not yet helped. (Antibiotics have never helped with any of my prostatitis problems!)

I just wondered if anyone else out there has had this BNI op and had similar issues afterwards. I didn't expect things to be plain sailing, and no doubt the wound still has some healing to do, but the frequency is a little frustrating. I had to go 4 times this morning in 2 hours! I'm drinking decaf coffee now but maybe I need to keep off that too. Bizarrely, I went out last night and had 3 pints and had no urgency to pee, and last weekend went out with some mates and didn't seem to need to pee anymore than anyone else. The frequency does seem to flare up and die down a bit, but I'm keeping a diary to try and spot patterns/trends. I'm returning to my uro in two weeks who said he would have to investigate further if things haven't improved by then.

Any thoughts/experiences would be gratefully received! Thanks very much for reading!

P.S. Incidentally, I've not has the dreaded retro I was warned might happen and sex is possible but still a little uncomfortable as things are very sensitive down there!. Fortunately, I have a very understanding wife!

    Nick sorry that it did not work.  Did your doctor tell you once you get prostatitis you can never get ride of it.  I have a friend that he has been deal with it for over 20 years.  He is only 33.  This is my opinion.  I think you let your doctor talk you into the surgery because of the symptom and he was a good salesman.  I think you just added to your problem.  I am deal with prostatitis right now.  Taking Ciprofloxacin 500 mg for the next month. 2X a day   My orgasm hurt more then it should and may also have a infection.  Yesterday was the worst.  Pain in the groin, seating was not good to much pressure on the bladder.  Left testicle pain.  Could not pee for 12 hour.  Took a hot bath and it seamed to work.  I would never get talk into having anything cut to get rid of prostatitis.  Even if a man have his prostate out he still will have prostatitis because it is in the surrounding area.  I hope things get better for you.  Your to young to deal with this crap  Take care  Ken

      Thanks Kenneth for your post. Just wanted to say my uro didn't talk me into it. That decision was mine, based on the fact that scans showing bladder was thickening, my bladder neck narrowing and I potentially faced damage to my kidneys. The frequency was also a big decisive factor. Also, just to give hope to others reading these posts, I had prostatitis for 5 years when i was in my late teens/early twenties and it went for 13 years without any pain whatsoever. Two years ago, it flared up again but even during this time, I had months where i had no symptoms at all. 

      I hope the Cipro works for you but sadly it never has for me. Saw Palmetto, Quercetin and Bromelain and Pumpkin Seed Oil however do seemed to have helped in the past. I agree with you about not cutting the prostate away. If it happens that the issue is pelvic floor pain, no amount of prostate removal will help that!

    Hi Nick,

    When I drink Beer, non alcohol or with alcohol everything is fine. It's the next morning when frequency becomes a problem. Spicy food or a combination of foods-caffeine, etc. can swell up Mr. Prostate for 24 hours or so. I suggest trying a water and veggie diet for a month or so to see if things settle down. Diet, heat, ibuprofen and flomax before bed has worked well. Did Uro provide measured prostate size (truss or Mri)? My prostate was 100cc. Prostatic artery embolization 09/20/2017, things are looking up.

      Thanks for your suggestion ktmxc, and for taking the time to reply to my post. Much appreciated. My uro hasn't measured my prostate, but he said it's small and feels normal! I do, however, have some significant calcification which I sometimes wonder might be causing the problem. My uro thinks not, however!

    Nick, I had a bladder neck resection a month ago following an unsuccessful urolift and recognize the problems you're experiencing. In fact right now I have a uti/prostatitis and a week into a course of bactrim forte it's still showing no signs of abating. Having to self catheterize which I now have to do just inflames everything even more. The resection I had was specifically ejaculate preserving by avoiding removal of any verumontanum tissue.

      Thanks for your reply makriebaba. I'm sorry you're experiencing similar problems. Yeap, antibiotics have never helped me with this problem, but maybe it's not bacterial. Either that or I'm just immune! Good luck mate - I hope you get some improvement soon.
