Prostatitis recovery period

Hi All,
So I've been trying to get rid of prostatitis for the last 3 and a half months now, hard to get a diagnosis but eventually got put on 6 weeks of meds (4 weeks levo, 2 weeks doxycycline). After 6 weeks of up and down in regards to progress, a few days after finishing the script the symptoms got better and urine flow improved (no more retention feeling etc.)
Read about only 5% have bacterial blah blah, but my doc seems to think all prostatitis is caused by bacteria at the beginning. Anyway, symptoms improving but I have also stopped drinking, smoking, eating spicy foods, sitting down for longs periods of time and cut down on sex. From your experience If I lapse and do these things will the prostatitis just come back?

Been battling this for nearly 5 years !!!! Yes 5 years !!! and although symptoms have reduced the condition remains :
Symptoms : Urethritis (from itchy to stabbing sensation), burning after urination, feels like a Grapefruit is wedged in my ass, pelvic pains, lower back tightness, urge to urinate more frequently, fatigue, a very dry throat (probably not related),
some flank pains (kidneys ?), Sore right teste (at times - normally at the start of a flare up), sore right groin (at times), coldness in the scrotum (due to blood directing to the prostate) etc......
Had muliple swabs (like 15 or so) and urine tests (20 or so) for Chalmydia, Gono, Trich, Ureaplamsa, mycoplasma, Adenovirus, E.coli, Staph, Strep etc........ all negative everytime.
PSA is normal , Prostate slightly enlarged to feel, no white/red blood cells in urine, no inflammation markers in blood.
Had cystoscopy 2 years ago - no findings however there was some irritation in urethra ????
Tried Doxy , Tinidazole (probably gave me most relief), Azithro, Cephalosporin, Amoxcyllin, Fluoroquinolones , Cipro etc........ Nothing sorted it but they did screw up my gastro system short term.Of all the meds the Tinidazole did give me some relief so I thought Trich may have been the culprit but then it came back and any microscopic inestigation never found the Trich parasite present in the Urethra.
Last week as a final plunge I went in for another cystoscopy (under sedation) except this time they squeezed the buggery out of the prostate to the point that I was p*ssing blood for a week. It was uncomfortable as hell but I hope they are now able to analyse the fluids and identify something.
Although not debilitating this thing is a "full time" nuissance that I am not sure if I will ever get rid of.
As a side note , too much sex , caffeine, Breads and cereals, excessive sitting worsen things, at least for me. If I am on my feett the condition is almost unnoticeable - sitting absolutely destroys me.
I am married and wife has no issues to speak of and has had routine blood & urine tets , pap smears etc..... No findings..
Will let you know if I find out anything from the cystoscopy (results early April).
I do believe this is all Bacteria or Candida related and the germ (for lack of a better word) has made its way to the prostate which is very acidic so Antibiotics are unable to penetrate properly.

Progress seems to be on the right side. PT indeed helped a lot, it turns out the pelvic floor muscles were badly damaged. For the past three years I was spending almost 10-14 hours sitting and a lot of it was on a plank type study table. On top of it I contracted a bacterial infection (so much for the good luck).
Strangely enough application of ice on the pubic,abdomen and pelvic muscles turned out to be more soothing than the warm sitz baths. Keeping off of sex and masturbation also helped a good deal.
As far as the stress issue goes even my uro told me that it could be a factor. And yes when i contracted the infection and all the other troubles I was going through one of the most stressful periods in my life (still not out of the woods though).
how are you guys doing ? hopefully all are recovering well.

How is everyone getting on?
I'm still having flare ups and trying to do as much yoga, relaxation as possible. Frequency down and heading in the right direction albeit with a grudging accetance that this may be a permanent fixture for the forseeable future. Still noticing that heavy weights affect it badly.
Any progress?

good to hear about your improvement and i agree those stubborn flare ups are a big stress originator.
The chronic pain and threat of the infection causing irreparable damage has thankfully ceased but continuous sitting does seem to cause discomfort. I think the infection did some serious damage to the pelvic muscles (I say that cause anti-biotics played a major role in eradicating symptoms in my case, so it's a fair conjecture that my troubles had their provenance in some sort of infection). Strangely i found regular running helped with the abdomenal discomfort.
I think max even i've come to terms wit the fact that this might be a permanent feature now. though I haven't had a major flare-up but it's never been like pre-infection stages. Yup the heavy weights do indeed cause some trouble. Relectanly, but i've made some lifestyle changes which includes weight training with moderate weights, reducing the alcohol intake etc.
I've been going through some really rough times so can't say stress levels are down but no more access to sex, that means Mr prostate has it wasy. :P
I've read about people making a full recovery, I hope everyone here falls in that category.

Thanks for the update.
I'm still dealing with the pelvic floor issues but do have them under control. Still feel them tensing up when under stress (due to my job this seems to be a common occurance) but keeping up the yoga, trying to relax and this does help.
Is anyone still on meds?

Any updates? I am freshly 24 and I think I have a form of prostatitis as well..
Had sex, a mixture of protected and unprotected events, which is what I bank on being the start of an infection. I had a gono and chlahmydia test after I ejaculated the following morning and felt like my semen "snagged" something on the way out, followed by burning urination.
Negative on the STI's, the following week I got sick of the dull irritation in my urethra and slight urination stream weaken. Doc did a urine sample, found no blood or other bacteria in the urine. Took cipro 500mg for 10 days. Didnylt really notice much, somedays I felt mostly fine and sometimes I would get hung up on the slight irritation.
Decided to masturbate after two weeks of abstaining. It felt pretty decent, no pain, just a little "off". Over the next four days I did it two more times with worse results, the last episode was three nights ago ane I still feel this dull ache like never before. And I have trouble uritlnating to finish, much much worse than pre ejaculations. My throat is also sore...
Going back to a urologist in two weeks, hoping to get some better answers on what tests we can perform. I really think a prostate massage would give me the best course to start recovery. I definitely think its bacterial..

Hey CJ, Max, everyone.
Thought i would chime in with my story. Based in USA not UK fyi. 32 y/o, 6 months of epididymitis and likely prostatitis, more or less CPPS though.
Had a sexual encounter with a random woman, and not sure whether there was any unprotected contact - we didn't fully have sex. But because i got a sore throat that night, i became incredibly stressed. I also masturbated 4 times that night, and the 4th time, nothing came out (i assume i depleted prostatic fluid but who knows, could have been fatigued peristaltic muscles in the vas deferens, or any number of things). I noticed a tiny scaly patch of skin on the glans of the penis the next day, and then i really got to freaking out (this skin patch disappeared in a couple weeks and i'll never know what it was -- maybe just sweat reaction, since i hid in bed for 3 days straight while freaking out).
I got tested 4-6 times over the following 4 months for Chlamydia and Gonorrhea but never tested positive. Did 2 urine cultures, a semen culture, NAATs for ureaplasma/mycoplasma, numerous standard STD Panels for all the major ones, and 3 blood tests for HSV-1 and HSV-2. Interestingly, i tested negative for HSV-1 2/3 times but the middle time i got "equivocal", and so at some point i'll do a Western Blot to get the most accurate result -- but it won't say much since 60% of people have HSV-1 and it's not usually a cause of epidymitis/prostatitis, but figures in other symptoms i'll describe.
So no positive tests for any bacteria. But had a crazy sequence of travel 2 weeks after my sexual encounter, and during a 12-hour flight (sitting on my prostate in economy+), i unloaded like a full cup of semen-like fluid while passing stool, and had to go the bathroom a second time to finish urinating and had extreme urgency. I had actually secreted a little bit of this semen like substance the day before traveling while passing stool as well, and it's only ever happened to me once in my life before, which was when i had tried abstaining from masturbating for 2 weeks. In this, it had also been 2 weeks since my 4-timer, and i in fact did abstain again for almost the full 2 weeks, but jerked off the night before my travel and it was the next day i started seeing the substance pour out. I'm assuming it was "congestive prostatitis" (you can Google it) and the flight made it worse, and the fluid was just prostatitic fluid.
Anyway, wound up in foreign country with the referred abdominal pain from walking around and left nut hitting against leg while wearing boxers. I mentioned the "discharge" and the doctors there didn't know how to do proper NAAT's so couldn't prove any infection but gave me the standard Ceftra IM + Doxy 7 days. I was riding a tour bus around the city which was probably horrible for my prostate and 2 days into the bus rides and doxy treatment i got horrible Epididymitis. My left nut was screaming in pain and the cord going to hid was hard as a rock. I chilled out until things softened up but to this day it's still sensitive and i avoid practically all forms of long-distance travel. I self-medicated an additional 2 grams of Azithromycin after that trip since things were still sensitive. I got stressed during that following week and that's when urinary intermittency started.
I then went to uro back home and got Bactrim for 5 weeks + Naproxin (NSAID) for the same duration. The naproxin did help restore urine flow which was great, but then started causing erectile pain, so i stopped taking it eventually. Fortunately, the urine flow stayed normal ever since, with the only issue being terminal dribbling (very little, but it's stayed with me for 6 months).
I went to a specialist in US who started educating me on CPPS. I've been reading "A Headache in the Pelvis" by the Wise-Anderson guys, who really invented the muscular relaxation therapy idea for CPPS.
All in all, i believe it was stress related. I can't rule out an infection, but much like CJ never actually tested positive for one. And the antibiotic courses never really helped, except that first course of Doxycycline which sort of helped but obviously not permanently.
I think it has something to do with having an orgasm while being extremely stressed at the same time. It must disturb the vas deferens peristalsis that locomotes the sperm from the epididymis up into the ejaculatory ducts and prostate or have some other effect in the larger pelvic area that sets everything off. I remember that night i jerked 4 times i got inner-right-thigh twitches right at the moment of ejaculation each time.
What's weird in my situation, is that i never got full on prostate pain. I had tingling in the anus, and it's definitely sensitive. Like if i walk 12k steps in a day, or run for a distance, all that motion of the bunghole makes it burn for a couple days -- which it never used to do.
But for me, the main symptom that remains is the epdidymitis. My left nut hands super low, and sometimes the ball seems to be floating in water and sort of atrophied. But then at other times it comes back to full life like the right one. Ultrasound showed decreased blood flow to the left testicle, and heterogeneity along the left epidiymis. The head of my epdidymis seems to always be super large on the left side. And also on the left side, the pain always starts with the tail of the epdidymis. Not really pain, but usually just sensitivity to sitting down to fast, or bumps while in a taxi etc, that hurt there and also cause referred pain to the abdomen.
There's also a little bump about 2 inches below the naval and 2 inches to the left, i've heard one other account of online, that sometimes gets a little hard and also often gets a red patch on the skin covering it. Swollen something -- lymph node, or possibly the section of the vas deferens that enters the inguinal canal.
Running helps a little bit, so long as i don't run too much. Travel kills it. Any buses, trains, planes (especially with turbulence) and even car rides destroy me. I now can only do short subway rides to work and back, and walking, and running. I literally am in the mindset that i will never travel again (well maybe once a year with lots of prep, NSAIDs, pillows etc) -- and this affects my career options as most executive roles require travel.
I am by nature a very pensive / paranoid / stressed person. And i have 2 other chronic diseases related to stress -- acholasia cardia, which is spasm of the persistaltic muslces along the esophagues, and pigment-dispersion glaucoma, which is sort of a closure of the ducts that reabsorb pigment in your eyes (and highly correlated with intelligent young males, believe it or not).
Now here's the weird thing --- in all of the forums on epdidymitis and prostatitis, the one thing i cannot find is any mention of lip symptoms. Is is just me that got these?
Ever since the whole fiasco began, i started getting redness around my lips. Sort of like peri-oral dermatitis. I've been suspecting that it's oral herpes, and maybe i got it from the same girl, but i've never gotten outright herpes with fluid-filled blisters that burst or anything like that. Just little red splotches around the vermillion border and upper lip etc. And it's often during masturbation or coincident with stressful times that also irritate the prostate/nuts. Or even happens during extreme sexual frustration when i try to abstain for a couple weeks. And just this last week, i actually got a tiny blister on the lip itself -- but not big enough, or accompanied by other blisters, to concretely suggest herpes. I've ended up giving up all of my regular face washes and lotions, and have to use Cetaphil only and soft Nivea face cream to avoid these symptoms. I can't really shave regularly so now let my facial hair grow out and just trim it. And i still get some redness every morning after showering that takes 20 mins to go away.
I found huge improvement from wearing jockey briefs. It's important that they weren't the Under Armor style boxer briefs that also hug the leg, because those cause the scrotum to move about every time you move your leg when you walk. So you the need the pure, free-hanging briefs that only wrap around the hip but don't have the portion that hugs the legs like boxer briefs.
The left nut never retracts quite as elastically as the right one. I don't know if it's just slow-healing damage from the initial inflammation, or a permanent fixture. But i think beating off usually makes it feel better for a day, but then almost worsens the sensitivity the following days. So for the last 2 weeks i've tried abstaining and kind of wants to try for 2-3 months (!!!).
It's amazing how different all our experiences have been. In my case, i don't get the "tennis ball in my butt" feeling everyone says prostatitis feels like, i only get the testicular tenderness. But because i got some urinary symptoms, i suppose i have to believe it is prostatitis still.
One time i tried doing intense ab crunches and that really hurt my nuts for like a whole week. So the abdomen-cremaster connection is something to take quite seriously. I got ocassional "tics" in the cremaster muscle / epididymis, often in the middle of the night during spontaneous erections, though they are rare and i feel like i've subconsciously "learned" to re-arrange the supporting muscles to avoid that reaction.
There're gotta be a myoneuropathic element to this whole issue. And i can't wait to figure it all out with the help of science. But my lip symptoms can be truly embarrassing and it puzzles me that not one else had the same thing happen. I feel like i errupted in a bunch of fordyce spots inside the lips as well and at the vermillion border, all around the same time, and paranoidly i think it was possibly related to my own paranoia about having herpes and like mentally "focusing" on the lips that almost caused it?? Sort of like how when you have a panic attack your hair can stand on its edge, there's this mental control of the follicles and sebacious glands in your body that happens through the parasympathic nervous system that maybe sometimes gets overloaded and causes blood vessels to inflame.
Anyway, that's my story, and look forward to learning from each other.

Hi Ahams, any update in your condition? My story is the exact same as yours right down to the lip condition. I too think it's mostly mental and my body is reacting to my overreaction about this whole thing. It's even started to effect my stomach, not sure if I'm developing an ulcer. I'm interested in hearing what developments or improvements you have (if any) .

I also had some oral symptoms when my "CPPS" or whatever this is started. I had super-chapped lips combined with burning in my mouth. It burnt out over time and is no longer a problem, but the CPPS has yet to let up.

I’m from the UK and currently relating to your early comments, I’ve done a lot of research and I’m convinced mine is non-bacterial prostatitis/CPPS (more pelvic muscle related than bacterial). I’m seeing a urologist soon but having to wait weeks on the NHS. I think mine came on via masturbation or really tensing my pelvic muscle area due to stress related issues. I’m really worried that the urinary symtoms are for life. I can’t stand the persistent urge.
My symptoms are:
-constant irritable urge to urinate that doesn’t subside and hasn’t subsided at all for 3 weeks (this is the worst!)
-a tingling pain 3/4 of an inch from the tip of my penis
-occasional left testicle, rectum, scrotum pain
-pain the the penis after sex
My main question to you is... did the constant irritable urge to urinate ever subside for you Max? As in, has your urge to urinate due to prostatitus/CPPS ever dissipated back to normal levels where you feel you don’t have that constant urge anymore? Also, was your prostate ever the culprit at all (I.e. bacteria/inflamed) or has it all been pelvic muscle realted for you?

Still grappling but getting there, as suspected I'm still dealing with a pelvic floor issue / pelvic tension. My last symptom is the urinary frequency which is increased by sitting. Not taking any drugs / antibitoics at present just doing the physical therapy. For about a week after treatment I feel fine and then the frequency and slight tension feelings creep back in.
In retrospect I do believe I had a bacterial infection (even if they didn't find anything) and now I'm dealing with this issue. For anyone out there with similar I can't stress enough that they should check out the physical state of their pelvis / lower back / pelvic floor as it may well be linked.
Weirdly one of my good mates who works in high pressured business and flies alot called me to say he had prostatitis so yeah seems this is to do with bad posture and stress (hence why they call it the truckers disease). Told him what I needed to hear 6 months ago...
I'm nearly there... hate saying that cause I almost always relapse when I get comfortable lol but yeah I am and so will you guys as well just keep an open mind about different treatments. Unfortunately as part of my job I have a to sit down at a desk a lot.

I'm being suffering form similar symptoms for about one month (pain in the right testicle, perineal docomfort, reffered pain to the medial thigh, somtimes to the knee and calf and there is also somtimes frequent urination).
I'm going through my final exams, so I tried to get rid off this problem very quickly during the last month by seeing multiple doctors and trying differed treatments for short periods and I failed and this made me depressed and more stressed out.
Finally, I found a urologist who diagnosed me with chronic postatitis and put me on "levofloxacin, doxycycline and finasteride" for three months. I'm on day six of treatment and still didn't find an improvment.
What can I do? any opinion? any Advice? please help. I wake up every day with my pain.
