Pruritus Ani - Long Term

I've suffered from an itchy anus for about two years now. I don't know why it's taken me this long to look into it .
What I know:
- When I first got it, I was using toilet paper fairly aggressively, and I'd frequently find blood on it
- In the last year or so, I've been less aggressive, and the bleeding is now very rare, however the itching hasn't abated at all
- The itching isn't just on one spot, but rather can flare up in several different places around my anus.
- While I am sexually active, all my STD tests came back negative
Is there any way I can determine if the answer is just a sensitive bottom rather than something like pinworms without seeing a doctor? And if it is a parasite, can you recommend any over the counter drugs to deal with it?

Always try a small area first and use your common sense but I have found it really helpfull.
It costs 3.50 in Holland and Barrett. It works well on broken or fractured bones, tendons etc.
I have just used it tonight on my sore wrist. Pain gone.

Then somebody who had recognised my discomfort, (noticed me trying to scratch secretly and often) told me I should just use witch hazel to clean myself after any bowel movement and before going to sleep. To my surprise it works!! I use moist toilet tissue and then cotton wool soaked in witch hazel after bowel movements and then use the witch hazel to clean before going to bed ....Nothing else ......it worked, no more secret scratching and better still no more being kept awake with the constant itch scratch itch scratch

1) Don't scratch--use an ice pack to relieve itching
2) Prevent irritation from wiping: using baby wipes (unscented, for sensitive skin) and water instead of toilet tissue, drying with hair dryer on "cool"
3) Increase stool firmness, reduce frequency of bowel movements to 1-2 per day: eat bananas and other sources of soluble fiber, slightly reduce insoluble fiber intake; within reason, resist the urge to poop
I also used Fire in the Hole spray, which I think helps relieve itching and helps healing, but I recommend only using it once or twice a day and drying (patting or hair-drying on cool) after using because otherwise it can increase irritation from the moisture.
