PSA SPIKE

About 1.5 years ago PSA score went from 4.5 to 9.0 in a year, MRI showed 103cc size prostate, no obvious signs of cancer.

Urologist started me on Dutasturide, was already on Flomax. After about 15 months on Dutasturide, PSA score gradually dropped to 5.1 and Free PSA score went up so Urologist agreed just keep monitoring as symptoms not bad at all, get up 1-2 times at night to urinate.

I get a PSA and Free PSA test every 6 months.

Most recent PSA score came out to 7.2 from 5.1 six months ago.

No change is symptoms so my plan was to wait until my next 6 month PSA test.

I am concerned about that spike from 5.1 to 7.2 but Free PSA stayed at 25 which is pretty good, no change from previous test.

Does anyone have anything to say about that, any experience with spikes in PSA that go up, do they ever spike back down?

Is this something to be very concerned about?

I still get up 1-2 times at night, slow stream when i go at night has been that way long time, but pretty normal stream during the day when active.

    PSA is an indicator of several factors, one of them being any prostate irritation. Therefore, it is recommended not to have sex or strenuous activity for 24h before the test.

    With your size of prostate you can expect PSA to be high. When on Dutasteride, the test result is about half of what it would be without. Depending on your age and other health conditions, you may need to start thinking about a surgery.

    Keep monitoring the PSA trend, if it keeps growing - have another MRI. Not a cause for concern after a single episode, but a cause for concern if there is a steady growth from one test to another.

    i have PSA tests every 6 months. Had score of 2.98 last December. 2 weeks ago had another test with score of 12.0 so agreed with GP another test now 7.8.Now due for another 3T MRI scan to see whats going on. Had an MRI 2.5 years ago with PSA 7.5 and got all clear. so PSA can jump around a bit.

      Thanks for that info, I had no idea it could jump around like that. I would be interested in hearing how it goes going forward with the MRI and such.

    Many meds can influence the PSA level.

    If the PSA is rising fast and you're off the meds THEN you should consider a MRI.
