Purple head, swells uncomfortably large during an erection, retracting foreskin makes it worse

I've visited the doctor a few times about this but since it only really happens during an erection, there wasn't much he could offer in the way of advice.

I've been given hydrocortisone cream to use for a week, which I have tried several times over the last few months to no effect. I visit the sexual health clinic yesterday and even they were satisfied that there was no sign of balanitis but it's definitely still a problem for me.

I'm not sexual active and I did have testicular cancer a few years ago so I'm wondering if there may be damage instead of a condition? Although I'm only 26 so I've ruled out a lot of things and I'm perfectly healthy otherwise. Idk, any advice lads?
