Doc wants to do 2 procedures during one surgery. I asked to revoke one procedure & not the other. They did. Should I revoke too in writing?

I called the doctors office and they canceled the consent that I had signed before for both surgeries. They said I would have to sign a brand-new one for just the one surgery. Which I did.  

Though should I still fax them a "revoke consent form" which I signed myself?  

They said it wasn't necessary because they destroy the old consent. And that I signed a new fresh one for just the one procedure.
