Random Spots all over Penis overnight.

About a week ago I had a bunch of scattered red dots appear below the head of my penis on the bottom side. I also had one small dark red/purple splotch on the head of my penis. At the time I did not have air conditioning in my apartment, so my groin area was pretty sweaty. I figured it was from that, so I ignored it and just kept washing up like normal and it went away after a day. Well fast forward a week, last night 6/15/2020 I went to bed at midnight. I woke up at 5am and the head of my penis erupted in purpelish/redish looking splotches. The same red spots on the bottom side appeared again, but this time they spread all the way around. Is this anything to cause major alarm? I don't understand how it can just pop up overnight. It literally looks like it's been there for months. I will get to a doctor but I was hoping someone could maybe give me some insight as to what is going on with my body. It doesn't seem painful but the area is a little sensitive now.

This morning when I woke up, my penis was erect and tucked under me weird, almost in a painful way. I'm wondering if maybe these are ruptured blood vessels???? image

    I also forgot to mention that I've had the same sexual partner for the past 3 years. I am in a homosexual relationship and I am on the receiving end, so my penis does not enter anyone. My S/O has been faithful so I doubt this is STI/STD

    Could be some allergic reaction or some infection. Consult a urologist or dermatologist for a proper evaluation and treatment.

    This is quite a severe reaction you have going on, from your sexual detail you give it would not seem to be possible - but this does look most like a bacterial infection.

    You need to get this treated as soon as possible - go, or contact your Regular / Family Doctor / GP / PCP, any Walk-In health facility or a Sexual Health Service provider

      You know what's very weird about the bacterial infection comment is that 6 years ago I slept with someone who had either Chlamydia or Gonorrhea(can't remember fully which) anyways I started having the milky discharge and burning sensation, so I went to the doctor and tested NEGATIVE for any STD. They treated it as an infection and gave me Doxycylcline, which after 7 days it went away BUT it keeps coming back randomly! Each time I test negative for an STD and they treat it as an infection. After 3 or 4 separate infections, they decided to give me a Z pack which cleared it up again but that was about 1 year or 2 ago! I feel like I got some rare strain of antibiotic resistant Chlamydia and it's hanging around but doesn't show up as an STD. I feel like the doctor does not listen to me when I tell them this, but why the hell do I keep having STD symptoms year after year but my blood work proves different?

    Hi peachy,

    Can you press a glass tumbler against the red spots and tell us whether the spots disappear? If they don't these blood spots are purpura (blood that's leaked out from blood vessels). Have you been unwell recently, e.g. with fever, sore throat? And are your joints painful or swollen?

    I suspect this is a small vessel skin vasculitis, probably Henoch Schonlein purpura (HSP). Go to Pubmed and enter pmc 1195093 in the search box for a case report of HSP involving the penis. You can also go to Dermnet NZ for more information. When you see your doctor, mention this diagnosis. He needs to do blood tests, check your BP and also your urine for any kidney involvement.
