Rash on back for a few months

hello! so my boyfriend has had this rash show up on his back and we cannot seem to get it to go away. we thought it might be heat rash due to our work environment, but it doesnt seem to be getting better. weve been trying to find a dermatologist that accepts his insurance but we're not having the greatest luck. anyone have any idea what it could be? the scratches are from him itching it and ive told him to try and not to but i guess it flares up. the photos should be in chronological order, the first one is from nov 18. its bumpy and feels kinda scaly. imageimageimageimage
