Received a pathology request form from gp and have no idea what to do with it ?

I recently visited the gp and was referred for a colonoscopy after 4 appointments and months of pain and also a pathology request form - immunology - faecal calprotcectin. The form is about half the size of an a4 size paper and just has information on it like my date of birth, gp details and small squares which I can peel off and put on something (assuming the container I send off in) . However I have no idea what to do with this form or how to actually request the procedure? Has anyone had this done I would love to hear from you .

    You need to get in touch with your GP surgery and ask them what the procedure is. You can’t be expected to know if no one has told you.

    When I got a fecal calprotectin test I didn’t know what to do with the stool container either, so don’t worry, you’re not alone in this! I started laughing when I asked the receptionist what to do because I had never seen these containers before.  

    She told me that should stick the label onto the container and write my date of birth and GP name on the spaces provided.  When I passed a stool, I was to collect some of it and put it into the container and seal it with the stopper provided.  I then handed it in to the receptionist so that it could be sent away.

    I don’t know what the small squares are though that you peel off.  I don’t know what you are describing .As far as I remember, it was simply a single label that you afix to the container.  If there are small squares to peel off, I would phone your surgery and ask about that.  That’s the only thing that puzzles me here.

    Unfortunately, receptionists simply assume you know what to do so they won’t automatically tell you unless you ask them to explain it.

    If it is not a label but a larger form, fill in the details required and put the form and your stool sample.in the plsstic bag provided.
