Recurrent cystocele

Hi all. I will be 5 months post laporscopic assisted vaginal hysterectomy with anterior repair and miduretheral sling on the 1st of December. My cystocele has reoccurred. I see my urologist next week so I don't know what stage it is yet but I think it's a stage 2. Has anyone had this happen and if so what has been recommended. I am experiencing same symptoms as before surgery. Pressure and discomfort like something is falling out. Feeling very depressed and disheartened.

I know how you are feeling. I had a hysterectomy & AR (no laparoscopy or sling) late 2013 and mine has returned. It's not as bad as before but the urogynaecologist said I also need a rectoenterocele repair. I believe the bowel was being held up by the prolapsed bladder. I have ongoing bowel problems since the first op so am reluctant to go through more surgery as the failure rate is quite high, but ...... many women on here have had more than one operation. Of course there are lots of women who have successful surgery who never need the support of a forum like this.
Have you tried a pessary? There are many different types & if your prolapse isn't too bad it might make all the difference. Specialist female physiotherapists are helpful too. Good luck.

I saw the urologist and he thinks I actually have an enterocele that's above/behind the anterior repair. Go back to my Gyn next week. It is not terribly painful just uncomfortable. Some days worse than others. Not something I would want to live with forever.

I believe an enterocele is in the back wall it's above where a rectocele would be????

From what I have read it's a herniation between the posterior wall of the vagina and the anterior wall of the rectum. Sounds like it would be above a rectocele but I am not sure. Feels very much the same as the uterine and bladder prolapse before my original surgery. Gets worse at the end of the day. Also feels uncomfortable sitting.

Update: my cysticele hasn't reoccurred. My urologist thinks it may be an enterocele. My gyn wants me to see one of his colleagues that has more experience with prolapse and the pelvic support structures. Some days it's not so bothersome but others it is quite uncomfortable. Go back January 11 for my next consultation. Does anyone else have any experience with enterocele?

I had an anterior repair 3 years ago. I now have a rectocele. Following an appointment with a urogynaecologist she says I need a rectoenterocele repair & my AR possibly needs doing again too!
My GP is trying to put me off having this repair as he thinks it's better for me to try & manage without it (I must admit I am quite scared to go through it all again).
I really don't know what to do as it is a big operation with quite a long recovery period but I suffer everyday with IBS/prolapse problems. I wrote to the urogynaecologist asking her to explain more about the operation & what the difference was between a rectocele & rectoenterocele repair. She replied that it was "medical terminology" & "not to worry". Well, I consider myself to be reasonably intelligent & was not impressed with her response! I'm still trying to decide if I should try & live with it or find another doctor.
Enterocele doesn't get mentioned much on here & I did ask if some information could be put on the site but I don't think this has happened yet. It might be that it's quite straightforward (let's hope so), unfortunately once you have one type of pelvic prolapse another is quite common.
Good luck for the 11th.

Thank you! I will update when I have seen the gyn. I don't think most of the male doctors understand how uncomfortable the feeling of something falling out can be.

I saw the gyn I was referred to and he says he sees nothing prolapsing. But I can feel it right above the scar tissue from the anterior repair which is where my urologist said he saw a prolapse that he thought may be an enterocoele. I have read that sometimes it can be difficult to see when you are laying down for a pelvic exam. I can feel a bulge when I am standing. Don't really know how to approach this with the gyn. Any advice would be helpful.

Unfortunately there is not a urogynocologist near where I live that I am aware of. There are gyn doctors that specialize in pelvic floor disorders but I have not heard good things about them. I don't think I am at a point where I would need further surgery yet but would like to know what I am dealing with. It's just very disheartening to be dealing with more prolapse after having repairs done. I work in healthcare and unfortunately my job still includes a lot of pushing and pulling. But I also am well aware that surgery is not always the best option.

Hi,
If you are worried about it could you go for a second opinion with a urogynaecologist? You may need to go out of your area if there isn't one close to you. From what I understand enterocele is the small bowel so I would assume it's usually higher up so may be more difficult to diagnose. My urogynaecologist only had a fairly brief poke about but she also had the results of other tests I'd had so I don't know what made her decide it was an enterocele as the gynaecologist & colorectal consultant I saw didn't mention it. Maybe it has developed later? If you think it could be an enterocele I would ask what tests are needed - if you find out let me know!
Send a PM to Matron she might be able to check out a specialist for you assuming you are in the UK. X

Update, I went for vaginal ultrasound today which was negative for any abnormalities. The gyn examined me again and this time also did a standing exam. He says that it's not an enterocoele but that the cystocele had reoccurred. Apparently it's prolapsed in an odd spot so it's possible I suppose that in the beginning when I was experiencing swelling that maybe I caused a stitch to pop. He does not recommend any treatment at this time because other than some discomfort things still holding. The prolapse he said is mild to moderate but realizes that's a matter of opinion. His advice is to be reasonable lifting and to come back if symptoms worsen. Most days it's tolerable. There's no issues with leaking and it's not at the vaginal opening yet. My plan is to be careful and hope for the best.
