Recurrent problem from 9 years ago or something more serious?

I am a white male, 27 years old, 5'10" 170lbs. Good shape, no other health issues, no medications. 9 years ago I was diagnosed with a hydrocele and varicocele. At the time I remember it being very alarming and painful before the diagnosis. What I remember from that event is that I had been masturbating to excess. Maybe 3, 4 times a day due to anxiety issues. At one particular session, I had ejaculated for the 3rd or 4th time in a day and experienced an excruciating pain in my left testicle. I went to the immediate care that night and they blew it off and gave me 500 mg naproxen (I'm not one to take otc or narcotic pain meds. I need to judge my pain myself to determine if the problem is better or worse, not mask it.) The next day I went to a different immediate care and they referred me to the ER. I didn't go and called my PCP in the am. That day PCP felt me up, felt weird stuff, and sent me to urologist, same day. Urologist and radiologist looked at ultrasound performed by urologist and decided hydrocele and varicocele. Sent home with more naproxen and an antibiotic (i think cipro?) I remember taking half coarse of cipro before blowing it off and not ever taking naproxen. I remember it cleared up on its own after a couple weeks. The pain is something like heavy, constant dull ache, sometimes sharp. Complete ED and hurts worse when aroused. Any arousal tended to hurt.

Fast forward to now, I still have anxiety and sexual issues, though much more leveled out. Was experiencing an extremely stressful situation in my life and was masturbating 2 or 3 times a day. Last Sunday I masturbated for the second time in that day(no ejaculation due to very strange feeling in testis) and my left testicle began to hurt. I tried to blow it off as the same thing, but had full blown anxiety attacks that whole day. I do want children, so knowing complications raise my anxiety. Monday I went into work and had panic attacks and left due to pain and panic. Called Dr when I got home at 830 when they opened. He said to see if things change or get worse, and he can refer me to urologist, past that he is no good. During our convo I told him I don't have health insurance. He called me stupid (jokingly) but I never saw a need for it as I hadn't had a health issue in 10 years. Now I work with heavy machinery and am a mechanic. Not always light duty work, but work had nothing to do with issue. I worked tues-fri after that with no problem except pain at night. In the morning my boys feel weird, but not painful, and move from no problem during day to pretty uncomfortable at night (6/10). Over this past weekend it has only gotten worse and now I decide I need to see a specialist. My fear and anxiety has been through the roof this entire week, and i wanted to wait to see if things would get better on their own. I cant find anything over the internet that describes what im feeling, but a lot of it is anxiety attacks and over googling, but still a decent amount of pain. I can feel swelling at the epididymis, and I can feel strong, coarse veins on the left side of testicle. No major swelling or atrophy of testicle, just ache ranging from dull to a pretty strong squeezing pain, but no discoloration of testes or urine. I tested my reflex, stroking inner thigh shows retraction of testicle. No torsion. Just pain and swelling at where epididymis meets my left testicle. I have no urinary symptoms. No blood, no extra need to pee, none of that. But what aggravated it worse was on Saturday my fiance (0% chance of std, we are engaged and serious. Absolutely.) tried to have sex and the arousal sent my problems into overdrive. I got erect but didn't even try for orgasm due to pain. A day later, now, I am stuck wide awake with anxiety, slight pain, and knowing even if I do get rec from Dr, it may be a while for specialist to see me. My boss is understanding but PTO only goes so far. This is my first post, and it is a desperate one at that. My fiancee has no idea how to relate and my pain isnt going away. She doesnt understand the sensitivity of the issue or the fact it might be a while before i'm seen by someone who knows. ER won't do much in my case, because if it isnt cancer or a torsion, they will just refer me to the same waiting list as my PCP. I'd be glad to talk to someone who has insight, or at least follow up to help someone down the road who might experience the same thing.

    Tl;dr Had testicular pain 9 years ago from overworking down there. Cleared up on its own, but was given ultrasound and they found a hydrocele and varicocele. Fast forward to now, was experiencing extremely stressful situation in my life and while masturbating, nearly same things happened again. I was seen by a nurse practitioner who works under my PCP. She ordered an ultrasound, done today, and it was read offsite by a radiologist who said its all good. No problems at all. But sitting here at home i can definitely feel discomfort and pain. I wasnt even given antibiotics. Just told to take NSAID of choice and wait for urology appointment, can cancel if things get better. It has been a week and day of same feeling down there, and my anxiety is better knowing it isnt blatant cancer or anything, but still no diagnosis of discomfort. Tylenol seemed to take the edge off of discomfort, but still isnt gone. Any insight would be appreciated.

      Just keeping updates for myself and for anyone who may have the same thing going on in the future. It has been 10 days since this started. The dull achey feeling isn't getting better, and notice it more during the day. At night I now have to take some sort of pain reliever even though I don't want to. Still complete ED and pain switching to discomfort and back and forth again. Urinalysis came back from lab saying no stds, no infection, etc. I am going to call Dr. Tomorrow morning and talk about lack of progress and hopefully at least start some sort of antibiotic?? I know all shows good on ultrasound and urinalysis, but have read too much about antibiotics helping some people who didn't show signs of infection. Plus am afraid of developing some sort of chronic testicular pain from not being treated. Any ideas?

    Just another update to anyone who might experience the same thing. It has been 14 days since the initial cause of pain. I guess the pain has been, very slowly, going away, though I still experience very weird feelings and movements in my balls, and occasional sharp pains and discomfort through day . I still don't want to try sex, and haven't. Though I have noticed sometimes, and throughout this whole time, my penis will reduce to a size much, much smaller than i've ever seen, while my scrotum will become more full and hard, but it still works, just scared to ejaculate. When I do try to get hard, it works, but I won't go further for fear of pain, then the rest of day it is super uncomfortable feeling. During the day my symptoms aren't so bad, though I feel a dull ache and sometimes pain, and at night it is worse. I stopped taking nsaids for pain, so that is better, but what is going on? I can still feel the swelling on my left epididymis, and I guess it is feeling better, even though haven't taken one antibiotic or hardly nsaid. I don't get why this happened in the first place?

      I would get a Lower abdominal ct scan and an mri of the spine. Maybe you have a herniated disc somewhere in your back that is touching the nerves and sending pain signals down there. Even if you have a form of IBS can cause this kind of pain.  Usually if you become constipated or even eat something that you have an intolerance too. It could cause inflammation that presses on the nerves.

      I appreciate the reply, but feel it is missing the mark a bit. I don't have symptoms of ibs, aside from lack of defication, which im sure is from not eating. Also, any lower back pain that i have experienced seems to be from sitting so much and trying to not make things worse, which coincidentally doesn't help. At this point with other emerging symptoms, I don't see or feel it as a neurological problem, but a urological or endocrine problem, and I guess partly psychological, as I get swept into these pain/anxiety cycles. But with no diagnosis, disrupting pain, and absolutely NO understanding from people around me, it has left me pretty bad.

    Just an update to anyone reading. It has been 17 days of this pain, and now it is evolving. The squeezing hot pain in left testicle does seem to be getting better, though inflammation of epididymus is still present. Also get weird aches in right testicle and found what feels like an enlarged vein on the back side of it. Now, however, within the past 2 days I have developed a burning, painful urination and extra pressure on bladder sometimes. When the pain flares up, it is like something is hurting my epididymus, along with something pushing on my bladder, causing my muscles in that region to retract, kinda like the same muscles that stop urine from flowing. I am also getting feelings like I am leaking, but i check and do the ol' post pee shake to see if i did leak, and i haven't actually got any wetness or discharge. Still can't eat, lost 15 lbs in 17 days. Urology appt. Is day after tomorrow, but the symptoms have me so worried and depressed. My life is not the same anymore, at least until the pain is gone. I cant sleep, eat, or even work. I am constantly in fear of the next flare up and pain associated with it. I was afraid of medical testing before all this, and never had my blood drawn. At this point I want them to do anything they can. Symptoms during day are testicle pain, and at night feel more like prostate pain. It has made my life a living hell revolving around pain, anxiety, and googling for some kind of answer. I will ask for second ultrasound from urologist, and maybe DRE? I have read articles, though, where is is a basic to avoid DRE if epididymitis is suspected. But with epididymitis I would assume that my symptoms would have gotten a lot worse by now from lack of antibiotics, not just changed. Regardless, one more day, then a couple more days before anything might start to help. I wouldn't wish this on my worst enemy, and have a bad feeling a lot of pain could have been alleviated by going to the right Dr. In the first place. Now idk what kind of damage is going on. I have also read how varicoceles can cause prostititus because the blood not flowing out of regular vein in testicle, causing the pressure, gets sent out to veins that the prostate and testicles share. Once this testosterone rich blood makes its way to prostate, can cause inflammation, because that's not where its supposed to go. Of coarse all lamens terms. I would really appreciate someone who has gone through the same pain to give there two cents. I really need some support.

    As some one who hashed chronic testicular issues , hydrocele both sides, verecele some also along w one other thing ......beware of surgery, after three unsuccessful ones I'm very Leary of any medical Specialists, especially urologist to diagnose, it's your body , even w tests, their 99% clueless guessers w surgeons license. Can't fix a busted pipe, can repair it! Major advice , exercise, nutrition , healing time, rest,sex yes.....be honest with fiance and so forth. It's not your fault yet.....but surgery can be bad. With...me then sewed the testicle too far center , and crowded my other to this moment! Now it's bending at apex and they say surgery again , forty yrs after original it's worse! Use exercise and nutrition!!!!!!

    Thank you for the reply. I have been trying to eat healthier and cut out as much alcohol as possible, but sometimes it is the only relief from the anxiety. I have been much too weak to try to exercise. My urologist was very cocky and not helpful at all in trying to explain any kind of prognosis. Basically just rubbed my aching testicles, wrote it off as epididymitis and possibly prostititis, didn't even bother to check prostate. Wrote me a script for levofloxacin 500mg 1 per day for 14 days, and gave me samples on phytotherapy pills for prostate. Been taking meds for 3 days now and I'm not sure what to think. I guess the ball pain and inflammation feels better, now it is just a super heavy, congested feeling in them, and prostate is neither here nor there. Sometimes super bothersome and I take a hot bath to help, other times I take strong ibuprofen to help. Now, though, the antibiotics are making it even more difficult to eat, and it is literally tearing my digestive system up. Been having crazy diarrhea, abdominal pain, tightness, hurts to bend forward. Just today noticed weird mucus and some red blood in mucus in stool. Still not sure if antibiotics are helping more than hurting. Tried to get an erection earlier. Had good blood flow, but was still too nervous to ejaculate. After that, pain and inflammation in scrotum went away after a couple hours, back down to the normal discomfort i've been feeling. Very difficult to get more than 1000 calories in diet, where before all this i could easily get 2, 3000 in a day no problem with no weight gain. Now i have been so weak it is hard to get out of bed and do anything. I am going to try to work tomorrow and call uro in morning to talk about stool/digestion issues. Does any of this sound normal??? It has me so messed up I am considering calling primary care in morning to do something about the unrelenting anxiety, depression, and weakness.

    I called primary care about blood in stool and he just said had to do with bad nutrition and antibiotics. I slept a total of 2 hours last night from pain. I went to work today and felt relatively normal, besides tired and a dull, aching pain in testicles (both now.) So I was feeling good and fine, and had no anxiety or anything, trying to get my life back. Then the same old thing happens. I get home around 4, then around 6 or 7 the prostate and peeing problems happen. I peed around 6 o clock tonight and it sent a burning sensation through my whole lower end. No blood, just pain. I'm on 4th day of antibiotic and still no real relief at night, besides an NSAID, and even then i wake up in the middle of the night with hotsweats and complete agony down there, everything in pelvic region. This has caused a major problem with my fiance and I. She is tired of hearing about my problems because apparently its no big deal according to urologist, yet i feel pain not associated to anxiety after I pee at night, even 4 days into antibiotic. I do drink beer, so I am starting to wonder if it is more fungal related than bacterial. Still no answers folks. Any one out there?

      Fungal is a definite possibility. I first had prostitis 9 months ago. Then went on a spree of sinus infections. Felt like s**t for the last 9 months. Finally I got a doctor to do blood work on Candida albicans. Results showed they were all to high. Waiting for my treatment options. Antibiotics make fungal and viral infection way worse.

      You are still waiting on treatment???Why didn't they give you an anti fungal then and there? What was all too high in your blood work? What kind of symptoms are you experiencing?

    Just another update. This one is just as full in detail as my others, so that maybe we can get more information about the male reproductive system. Obviously little is known about it when it comes to pain, and if problems persist after antibiotics, most specialists are useless. Read my other posts for more info. This morning i woke up tired, totally exhausted, after getting 7 hrs sleep. I remember waking up 2 or 3 times from pelvic pain, congestion, twisting feeling, whatever you wanna call it, but no pee. Pretty much every night I try to ignore it. Today I was working and felt the need to deficate. I went and noticed my righty had completely sunk up into me. I had to coax it to come back down into scrotum. After that I felt ball pain in right, instead of left, but when I get home and have a couple beers, my stuff hurts again, then I take ibuprofen. Every day I take fish oil, garlic turmeric and antibiotic. I take ibuprofin at night to ease swelling and pain I get once I get home. That and a hot sitz bath help. I stopped taking phytotherapy for prostate because it doesn't seem to help after 5 days. Why is this every single day and every night? Why has it rendered my sex life useless? I am assuming it is a non bacterial cause. Why are doctors useless in this part of pain??

      Maybe it does, maybe it doesn't have a lot to do with it, but I do drink a fair amount of beer at night. It has never in my life affected my urination at night, nor caused any symptom. I have been drinking more at night to relieve anxiety from pain, but have noticed an increase of pain during urination after drinking. I tried to not drink for a few days and just got insomnia, along with pain, and pain being the major factor in the insomnia. I guess this is my outcry for someone who has beat this. I just want my life back.

    Another update for anyone who cares to read or share with their same experience. I have been on Levo 500mg 1 daily for 8 days and it is nearly impossible to say if things are getting better or not. The inflammation and discomfort are still present in left testicle, at the outer top area, and rear of right testicle towards the middle. Discomfort is always worse at night, and if I physically touch where the inflammation is, it hurts worse. The prostate pains seem to be doing slightly better, but with wide variance per day. Sex is still impossible, though my libido is off the charts high. I have read that symptoms typically improve after a couple days of antibiotics, but in my case, if they even are getting better (or maybe i'm just getting more used to symptoms) it is an excruciatingly slow process. Is this because I had to wait 2 weeks to be put on antibiotics? When should I contact my urologist about it? Has anyone had to be put on months of antibiotics for epididymitis and it actually work? Given that STD is impossible and they are treating it as a bacterial infection, is levo best option? If it happened to be chemical from urine, is levo best antibiotic for such a case? Is there any hope??? Should i go back for a second ultrasound? Just don't understand why after 8 days I can't sit here and say "my symptoms are improving." I am also concerned about lack of nocturnal emission. Going from ejaculationing 5-12 times per week to absolutely none for nearly a month, shouldn't my semen be looking for a way out? I have been tempted to ejaculate just to see, but too fearful for A: what if it all starts back over again and I seriously hurt myself (pain started just before ejaculation first time) and B: too afraid to see blood or anything else in sperm, or lack of sperm overall. Someone's gotta have some input... Anyone?

      Of coarse, I was getting more relaxed about my prostate symptoms but as I sit here now, the prostate problem is starting to happen again. Seems like some days it almost goes away, then others, always around this time, they come back. Tonight, very often, and painful urination, and feeling like I'm sitting on a golf ball. Please, I need help. I can't go on like this, especially if nothing ever works. Sometimes I would rather just paralyze my lower body, and get rid of all the pain at once.
