Red foreskin and glans of penis + two shiny very red dots on glans

I have been having cuts and two shiny red dots on the head of penis for more than 5 years. my foreskin is also extremely red,especially around the scrotum. redness of penis gets worse when penis gets hard. obviously masturbation makes the penis extremely red. i have tried so many creams already but none of them work: betamethasone, advantan, tacrolimus, clotrimazole, fucidin, and flucanozole. doctors dont want to prescribe any other creams. also they dont know what skin issue i have. i dont know what i can do about this (besides going to doctors who dont know what to do). please let me know what you think. i feel like i cant get rid if this
