It's probably nothing to worry about, but it's hard to say without pics. This should help you add images:

If you're using a phone or tablet, you may need to choose the desktop/PC option in your browser.
If you don't want to post a pic, then please provide more details. The urethra is actually the tube inside the penis, so what exactly is bright red and puffy, and where are the spots?

It might be worth trying an anti-fungal cream similar to athlete's foot cream for a few days. Should do no harm and might improve your skin. If not, see a doctor!

Those do look unusual - maybe some form of balanitis?? Probably harmless, but I'd get them checked out. Do you have any skin conditions elsewhere eg eczema, or psoriasis?
Some of these may help:
take a leak and wash down there at least once a day, and always after masturbation
only use water to wash down there (no soap or shower gel)
Canesten cream
In your position, I'd see a doctor though. Hugely embarrassing I know, but You could either see your GP, or if you're in the UK, you could go to a genitourinary medicine (GUM) clinic - google "GUM clinic near me" for details. They're not allowed to tell your parents you've been, and you don't even have to use your real name. Some places have clinics for under-25s.
If you could update this thread and tell us what the doctors say, it would be appreciated. Good luck.
Disclaimer: I'm not a doctor.

missed a bit:
Hugely embarrassing I know, but too many males don't see doctors about issues down there because they're embarrassed. Sure, it might not be an enjoyeable experience, but it's your health we're talking about, and this may not be something that just fixes itself. Doctors have seen everything, including stuff muuuch more embarrassing than what you have. Either you have something that needs treatment, in which case you should see a doctor sooner rather than later, or you'll find out that it doesn't need treatment and you'll be able to stop worrying about it.
As I often point out here, women have to lie on their backs with their legs in the air on a regular basis, while a stranger looks inside them with a speculum and flashlight, so we males have it easy. There are way too many men who won't see a doctor till they absolutely have to, and it's something we need to change.
