Reducing sertraline

I've been taking 100mg of sertraline for a year or so to relieve anxiety and periods of depression. I started oral contraceptive also as was diagnosed with Premenstrual syndrome, and that has helped balanced the hormonal fluctuations. I would like to reduce the sertaline gradually as i've had some unpleasant side effects (nightmares) but other than prescribing a lower dose of 50mg and telling me to alternate I haven't had a long term plan from gp on how to gradually reduce, assess and eventually (hopefully) come off them, or any other dietary adjustments I could make to support change. I've written up a 18 month of alternating month by month to a lower dosage of 25mg by summer 2025 and then hopefully stop. I went cold turkey before and had severe side effects so am very wry about doing it very gradually, but this was before I started taking the oral contraceptive so hopefully I won't have such huge hormonal fluctuations. Just looking for any advice on what other people have done. thanks.
